We are constantly bombarded by automatic thoughts which often dictate our underlying emotions. Learn the skill of noticing your automatic thoughts so your not a stuck with automatic negative emotions.
"mindfulness if the dot" is drawing your attention to a point of focus- the task is to notice when we are distracted from the point of focus by our competing thoughts. noticing the thoughts that bubble up rather than getting lost in the thoughts. we need to cultivate our awareness of our thoughts: metacognitive awareness

I desperately wish to handle my anxiety with skills. But I’m often in a state of unreasoned terror, which doesn’t occur because of thoughts and may descend the moment I wake up, without any prior thought or even true consciousness - and it doesn’t seem to be penetrable by any skill. Even Valium, means of last resort (which I normally never take) doesn’t make a dent in it. Ice cold showers make a couple of minutes dent in it. It is so awful that it makes me immediately suicidal. Note that I’m not an impulsive person at all, have never made a suicide attempt, don’t engage in self harm, etc. It feels like a danger to my life, and I am out of ideas of how to treat it (very healthy living, strict bed times, gym, nothing has helped.)

@I. Ehrenfest - It might be help to look into TIPP technique and vagal nerve stimulation techniques. What you describe is common in PTSD. So if trauma is relevant, getting some help with that might help silence the alarm. Other things to explore with your doctor if the aforementioned don’t help: clonidine, beta blockers, stellate ganglion block and transcutaneous vagal nerve stimulator.
