Dr Rich Blake, for drblakeshealingsole, describes 8 orthotic modifications to help block the tendency to supinate too much. This also called lateral instability and is responsible for at least 17 different pain syndromes.

Walking for me is like an Olympic Sport....due to reconstruction of leg/ankle , car accident . It’s many years later but still hopefully someone will make an orthotic for me that helps. So far I’ve been able to “make” my own similar to this video, definitely glad I found this video , I now have confirmation I’m on the right track!

Its sad, I'm 59 years old. I've work hard for at least 40 of them, always on my feet. I've been on disability for 3 years due to chronic back pain and a lot more. So many c/o back pain to the Dr's and not one checked my gait or stance. I'm upset. Should I be. And that for all the time and effort u put in to this

That explains my hip..knee.. sprains..foot problems.. I'm stuck in this state since I am unable to afford the right shies.. let alone the proper orthotics for my situation and to keep up with it for the next 40 + years.

Dr. Blake Thank you for sharing your knowledge with your readers. Hope that you may be able to assist me. Have Metatarsaligia - left side and Excessive Supination on the right foot only. Also “enjoy” IT Band pain on the right as well. My Pedorthist created orthotics for the Metatarsaligia issue. However after 2 modifications they are very painful and can not wear them. Would appreciate your comments.

I always wondered why I had calluses along the outside length of my left foot and on the inside of my right big toe. I do have very high arches but apparently my left foot over supinates, and my right foot over pronates. I stacked 2 high quality arch supports in my right shoe and voilà! my hip pain vanished immediately. The effect was to pull me to the right and prevented me from leaning to the left. I'm heading for the podiatrist!

I think I have some moderate supination. I recently developed a lot of lateral pain on the outside and underneath the outside of my foot. I've been wearing Powerstep Pinnacle blue inserts. Are these inserts helpful or harmful for my issues? Thanks very much.

My wife suffers from plantar fasciitis as well as pain in her piriformis muscles. Wearing regular sneakers with orthotics rectifies the situation. She just started to play Pickleball and wears tennis sneakers with her orthotics. Unfortunately, due to the tennis sneaker's flat sole and small heel stack, she feels pain in her piriformis muscles and Achilles tendon. Her orthotics have a 2 cm. heel stack. Can this be safely raised higher to alleviate her discomfort while playing. If not, what else can you suggest? Thank you so much.

This is the first time I found this. My doctor had orthotics made for me telling me I had flat feet. However it made my feet worse...I felt like I was walking on the outsides (lateral side) more than normal. I've been told by several doctors now that I have repeatedly sprained my ankles and I have all sorts of issues. I've never been aware of an ankle sprain. Anyhow, I showed the doctor who made the orthotics the bottoms of my shoes b/c he didn't believe that I was suppinating. When he saw that he couldn't believe it. Apparently, it is rare. So he glued a sponge to it and it fell off 3 days later. I was so ticked off I never went back. I'm going to try and fix my orthotics to look more like these and see if they help!!!

Hi Dr Blake. Thanks for posting this - this is very useful to me. I am getting ankle, knee and hip problems due to a supination/foot twist similar to the one you describe (birth/bone defect: my heels kinda roll inwards, so to speak, as though an outward triangle/chunk of it was missing so to properly support the ankle). Sadly, I could never afford to have myself an insole designed specifically for my foot. So I got myself a baseline (an insole with a deep heel cup and a medium-high arch similar to the one you describe, sturdy/cushy enough it seems to me), and would like to try some DIY add ons to it, that would "fill in" a homemade E+F (or A+B) Denton supplementation, if I may express myself that way. I was wondering if you'd have some suggestions as of which material and/or fixative to use ... ? The grey material you are using on part F of your design, got me curious in that respect - what is it? More generally - any dos and don't for the "starving supinator artist" out there... ? Thanks - many, thanks to you in advance!

Marie-Nathalie, thanks for the comment. Try the insole material from old running shoes. I use barge cement but superglue in small drops spread through out will do. You just cut the section out and apply to the Denton or valgus wedge size as in the video. Hope it helps. Rich

I went to two local podiatrists. One had me buy expensive orthotics that caused so much pain I quit after two weeks, it is a year later and my feet are still in pain from them. The other one told me I needed to lose weight. Unfortunately, I'm on a medication that packs on pounds. I've tried all sorts of inserts and cannot find any that help walking on the outsides of my feet. I can wear out the heels of shoes in a very short period of time, including shoes like SAS, that are very supportive, but the soles wear down fast. I have IT band trouble and horrible SI joint pain issues for the last 8 years. I have an extremely high arch, as well.

I do not know how to help. Did the orthotics you got, even though painful, help with the supination. If they did that is a good first step, now you have to figure out how to make more comfortable. Please answer back. Rich

@@RichBlake No, actually, I'm having trouble standing on anything hard, so there was so much pain that started from those orthotics, I could barely walk, so I don't believe I could tell if they helped the supination.

@@barbaramazzocca4605 The approach that they could use are soft based inserts with no supination correction at first to make them comfortable, and then to gradually add anti-supination modifications underneath. The part against your foot would remain very soft. Rich

I have Progressive systemic Scleroderma which gives me excessive over supination. The insoles in this video are exactly what I need. I have the hardest time with balance and I've broken my feet just walking. How can I get these insoles? They would greatly improve my life.

Shelly, thanks for your comment. These are made by podiatrists with a biomechanics background. You can call the Root Functional Orthotic Laboratory in California and find out who in your area makes these. Have them look at the video to see what is necessary before giving you a recommendation, and then show it to the doctor who will be ordering the orthotic device. It can take several tries to learn what you need to stabilize your foot. Good luck. Rich

Hi. I have recurrent Peroneal Tendonitis which I believe is directly correlated to wearing Reebok Easytone Instability exercise shoes for many years. I’ve been told that my muscles are not working together correctly and too much pressure is being placed on the ankle and foot from tight calves. I am currently in Trigger point therapy, however, I have a pair of orthotics that have a deep heel cup and lateral wedge. It’s after the break in period, and my big toe and medial ball of my foot is bright red. While walking, it is uncomfortable and gets painful on the lateral side. Peroneal Tendons are great. Does this mean the lateral wedge is too high or should be removed? I’m wondering why I have new pain that isn’t part of my existing condition.

Yes, please have 1/2 of the thickness of the lateral wedge removed to see if you feel better. The orthotic should get you in the center of your foot, not too far medially (towards the big toe joint).

Do you know of anyone in central Illinois that is capable of constructing a brace similar to the one you described, I am 14 years post stroke and ove supination, hyperextended knee a toes curl

Dear Doctor, I am about to get my orthotics made on this Thursday, July 2. I am afraid that I might offend the doctor by suggesting these modifications to my orthotics. Is there some way that I might make these suggestions without appearing to be an untrained person speaking to a DPM? I seriously doubt that the doctor will make these modifications. Do you have any suggestions?

TheMenghi1 Sports Medicine is a team approach to treatment with the patient having equal weight in decision making. I never mind the patient making suggestions, like reviewing a RU-vid video. Tell the doc you find this video from a semi-reputable podiatrist to look at and show the lab. The orthos being made may be perfect, and you were just being cautious. However, if the orthotics do not help control the supination, ask if the lab could redo with some or all of these modifications. Most docs are pretty open to this in face of failure, except for a few!! Hope this helps. Rich

Most doctors who are interested in biomechanics know of Richard Blake and would take his suggestions quite well. You (or the next person with this question) should also give your doctor the URL and suggest they watch the video. The description is clear and logical for all of Dr. Blake's suggestions. While all may not be necessary for a particular patient, they all should be understood by the Doc. Note: over a year ago a patient who was also a physician pointed out this video and asked if I could add a few more of the steps described here into his orthotic. I was happy to do so.

GOOD to know it can be modified back in the lab. I have the luck of having only one foot with issue. I did a MacGyver ~ Denton Modification and OMG. Taking it in Monday with the link.

Would this type of device work for someone who had a cerebral haemorrhage related neuromuscular foot problem? The person had the cerebral haemorrhage 10 years ago and she is now 34 years old, and only her left foot is problematic. She has not developed a foot drop but she tends to supinate her foot during her gait. She says she is unable to control it with her brain and is also getting help from a physical therapist for muscle strengthening. I was wondering if this type of device designed for over supinators would work to retrain her brain to pronate her feet. Her arch height is just average - not flat or overly high yet on stance she is trying to grab on to the floor with flexors in toes.

I too have had custom made foot insoles and not only did they all fail, they also caused terrible pain in my feet, knees and back. They are all too hard. The simple drugstore insoles tend to be better because they are more flexible. Why don't these food doctors understand the concept of soft and flexible? Why don't they create insoles that try to gradually corrects the problem as oppose to all at once with one insole which is too much of a radical change for someone who has a problem that has developed over years. It's like someone who has not bench pressed weights in years trying to bench press 200 pounds. There are also foot exercises for foot conditions that are probably the most critical thing that a person can do along with swimming and hot foot soaks with epson salt.

Some of it is that patients may not get better until they have over 80% correction, so the longer you wait the more the problem can get out of control. Some of my patients definitely get soft based orthotics for shock absorption and cushion, but they are always breaking down. When I design a pair of orthotic devices, I try to access the current needs of support vs cushion. When I guess wrong, and the patient needs something else, I typically has plan B. Richard Blake Dr. Blake my criticism was not directed at you specifically, but summation of my experiences and what I have heard of others. After doing my own research I learned that I have both splay foot and over supination in my right foot. I now know that there are soft foot insoles that can help correct the problem and that there are specific exercises for both conditions that help. After nearly ten years of suffering, and experiencing pain with every step and unable to run or even walk normal not one of the many foot doctors that I saw ever mentioned splayer foot or over supination! Not One! The all wanted x-rays, diagnosed arthritis, put me through physical therapy that was worthlesss including the electrical stimulation and two minute foot massage and of course their expensive hard and worthless foot insoles. I am doing much better based on the things I learned on my own which also includes regular swimming. So why didn't all of these doctors didn't tell me what any high school kid can find on the Internet?

Walking is so painful. In my medial knee(right side) i walk outside only on that side. Strange enough running doesnt hurt. I think cause i strike different. Can my medial knee hurt from this?

Hey Nusrat, hopefully you can work with a local podiatrist or physical therapist. They use professional orthotic laboratories which can manufacture these off an impression cast of your foot. Rich

Many thanks, I been tryin to find out about "how long do memory foam insoles last?" for a while now, and I think this has helped. Ever heard of - Danandonton Miraculous Domination - (Have a quick look on google cant remember the place now ) ? It is an awesome one of a kind product for discovering how to end your foot pain minus the hard work. Ive heard some awesome things about it and my mate got excellent results with it.
