I can't fathom an ER (or ICU for that matter) without an ultrasound. it is absolutely indispensable. Line placement , cardiac visualization, blood flow, L&D are just the tip of the iceberg. I can think a hundred other things that can save lives when you have it.

Bedside ultrasound is the BEST. I used it alllll the time in the ER and have been begging for it in my urgent care! It would help prevent us from having to send almost every patient with abdominal pain to the ER.

I used to work on algorithms for improving the image quality of non-contrast point-of-care ultrasound. It’s wonderful to see my work making a difference.

Back in my ICU days ultrasound of the eye was suggested to look for increased cranial pressure by scanning the optic nerve sheath. A diameter greater than 5 mm was supposed to be indicative of ICP elevation. Never did it or seen it done in my practice but our patient population was not the type that would have elevated ICP.

Regarding brain ultrasounds, my understanding is that it's common enough in NICU babies to look through the fontanelles to see what's going on there. My daughter was diagnosed with periventricular leucomacia, basically cysts in the brain. She's fine now, but it was extremely scary for us at the time.

From a patient perspective, I am so glad that ultrasounds are part of ED care. Its non invasive. I did my EMT training in the 19080s when they still did the abdominal wash to check for bleeding. OUCH!

I've seen a team of french resaerchers developping a 3D ultrasound technology which is very precise because they use micro air bubble that they put in the blood stream and the interference of the bubbles allows the ultrasound machine to track the blood flow with great precision and IIRC they also had a software automatically coloring what they wanted to see in color.

Ultrasound as become so portable and so affordable and so powerful that a lot of EMS systems are starting to add it to their toolkit. Initially it's been used for cardiac arrest and especially for COVID locating consolidations in conjunction with a phone consult from the ED, but it's starting to be talked about as being a sort of killer app for community paramedics and treat-in-place protocols in general. It's actually kinda nuts in the best possible way

Oh my goodness. I'm so glad you brought this episode. I was in the emergency this year one to get my vein checked cause it bulged oddly (post my surgery visit(s) and there was basically an injury of the vein valve due to the intravenous catheter) and my doc wanted to make sure it's not a blood cloth). Now, my appointment was laste so I came to the ER late. And the doctor was unhappy with me, cause by the time it was my turn, the specialists were all home, so to speak (Germany, and it's a small town hospital). And I was confused. Because using ultrasound is like ... the most common thing ever. It's everywhere. But that person seemed unhappy to have to use it. :'D Now that I learned that it wasn't necessarily all that common - back when they got their training - it starts to make sense. Yeah, obviously that would be a big deal for some! And he made a grumpy side comment towards "German doctors" and you know .... people having lives and going home, as I corrected him. :'D I'm so used to ultrasound, especially from the gynaecologist etc. that it never occurred to me that method may not have been that popular such a relatively short while ago. And that it's not necessarily part of the standard things taught to all doctors at the beginning. Ooops!

If I'm correct, I believe Imaging, to include Ultrasound, is the leading revenue generator in a medical center. Followed by Surgery.

Dude it's good for intimidating interns and residents how did you miss that one. I'm pretty sure that's for a ticket to your show if you come to Ohio. Breath sounds are always diminished in the lower lobes unless you have an eko stethoscope. So powerful and efficient even pulmonary patients can be thoroughly assessed just by listening to the at the triple point. CTA@TP PT chronic emphysema stable.... Edit .... Ummm... I'm actually shocked anybody needs to convince doctors to use ultrasound. Where I live, USG is as instrumental, elementary and obvious as a stethoscope or a neurologist's hammer. Like, duuhh.
