There are 3 key pain spots for pain from a rotator cuff tear.
Other areas in the shoulder may hurt, but they can be due to different types of shoulder injuries.
Knowing what pain in each area means can help you figure out if you have a rotator cuff injury or if your shoulder pain is coming from a different type of injury.
The treatment you choose to do may differ depending on what type of injury you have.
In this video, Dr. David answers the question, "Where should the shoulder hurt if you have a rotator cuff injury?"

Hello, Omg, you just made my day. I have everything you described. I have 2 full supraspinatus tears on both shoulders . I have pain In all the areas you describe. Imaging also showed some impingement and a r sided downslope scapula. My ortho said the pain/discomfort from the crease under my elbow to my hand/fingers(tingling) has nothing to do w/the tears. I didn’t believe him b/c I had already researched it. I swear doctors are so pigeonholed . They Don’t even know how the muscles can work and what trigger points can do . Your video nailed it !! My neck is definitely involved, my scalenes and upper traps. I traveled to see a sports medicine doc and he told me my scalenes , feel like an extra clavicle/dense, left side only. When I told him that I had actually two full tears now in each shoulder he said there’s no way that there isn’t something else going on. He suggested getting Botox into that muscle to poss release it. The PT’s, majority of them , say to stretch the upper traps, very rarely to strengthen them. I haven’t tried your exercises yet, but I do know when I’ve tried before to strengthen my upper traps , my left muscle/s spasms and bothers my neck. I don’t have any pain on my right side though I have a tear there but no issues yet. The doc mentioned I could go ahead and do PT to try and strengthen the muscles and surrounding structures since I still have full use of my right arm and my left arm I have about 80%. I just have more pain/discomfort than anything. I know full tears don’t repair themselves but if I could strengthen everything to do my best to get strong. I also have issues that I’ve been having issues over the past years my posterior shoulder muscles I’ve been trying to strengthen them and something is inhibiting it, so that’s another issue. I don’t know if you will answer this, I know you haven’t seen me, but would it make sense also to work on my scalenes first to massage and stretch I know a lot of time muscles are tight because they’re weak. I don’t know which one it is . I know I’ve had scalene issues for at least 10 years now but not the rest of the stuff. Thank you! Also, thank you for your video!!

Working on home “boat”physical therapy. I live aboard a boat and was able to see a physical therapist the month of January. We did blood flow restriction, hyperbaric chamber and one on one for an hour with the physical therapist. Currently no option for PT office visit in the Keys. My question is I wake up with my arm over my head. Not sure if my body finds this position a better sleep placement for my arm. Thanks for the great videos. When the pain is intense after working lines or a rough day on the water I always go back to the beginning of the arm on the pillow and cat/cow. It’s a wonderful way for me to reset and decrease the pain level. Once everything calms down I go through my prescribed exercises.

I had a pain the other night that went from my neck/ 4:00 shoulder, down my arm to my elbow and through my hand and out my fingertips. I screamed - the pain was off the chart. Have not had an xray or MRI.

Hi , I have a partial thickness subscapularis tendon tear which is very painful and restricted mobility. My PT has told me to do isometric exercises during phase 1. No improvement though. My question is do you think I may benefit for your 5 top exercises you recommend

So I cant reach behind me or "stand-at-ease" or stand up and do the lucky cat motion like you mentioned in your other videos but don't have pain in those 3 spots ,. I do have a single painful spot when trying these aforementioned motions reaching for something or lifting something (of some weight) with out-stretched arm . Would this mostly be a tear or tendinitis or impignement in your opinion?? Would be helpful to know so I can start the correct type of exercises .

I have stopped using a pillow at night. And cut my long Dreadlocks. I have noticed hair loss and the urgent need to crack my neck multiple times a day. I have stiffness that radiates from my shoulder blades up to the best of my head. Do you have any recommendations?
