I’m a doctor working in the system , this guy has NO idea what he’s taking about. People don’t consume health care like any other service,they have no idea what things cost, they don’t have time to shop THEY JUST WANT TO STAY ALIVE,

@@robert5897 I was making a joke about how in his insane fantasy world people will be rational consumers of healthcare -- just one, of many, wrong underlying assumptions for his Capitalist Utopia to even be capable of existing. Now we could debate the economics of healthcare, but I am sure that, like the video, it will devolve into the dismantling of the libertarian ideology with the simple statement: Capitalism cannot exist without the state enforcing property rights.

@@robert5897 You want to talk economics? The theory of rational behavior is shit. It sounds nice, until you look at the real world. People consistently make decisions that don't maximize their utility (e.g., Americans actively make that decision when they vote for politicians that ruin their country). Tbh the whole theory of rational self-interest is trash. Psychology teaches us that we are full of contradictions (you want to do some things and don't want them at the same time, etc.). Also, see dialectics. Everyone is ideological, but let's not get into that. Yes, it was in bad faith; I'll give you that. Then answer the question: how can capitalism exist without a state to enforce property rights?

just so you guys know, we singaporeans also have a forced savings plan (CPF) controlled by the govt. it's a pretty hefty 20% of employee's wages, along with a further 17% contribution from the employer (i.e. a total of 37% of paycheck goes into our CPF accounts by law). the CPF functions both as a sort of universal insurance covering pretty much the entire population (the coverage is also called medicare/medicaid here) and a savings account with high interest rates but heavily limited "withdrawal" (we can essentially only use CPF money to pay for public housing and healthcare costs above basic coverage). while it is a beneficial programme overall, it is also a huge amount of the country's wealth being directly controlled by the govt, with very little alternative for the citizens - there is practically no opt-out of CPF contribution, short of changing jobs to become self-employed. in short, singapore's CPF is 100% unacceptable to libertarians.

Nothing in our current system stops charities from paying for sick people's medical expenses AND these inadequately funded sick people are easy to find; about 1/3 of GoFundMe are people needing help with medical expenses. Why aren't the charities working? And should we trust them to work in the future? Why?

And if letting 40k Americans die in pain because they lack money is considered moral in your view, then we fundamentally disagree on morality. I think saving lives is more moral than letting people die. Also, what part of my idea is theft? Please note that I haven't said anything about my idea, and my plan does requires that there is ABSOLUTELY NO TAXATION. So please, educate me on my theft and first amendment violations.

Your avg patient does not have the in depth knowledge to determine best practice for a procedure, a point Sam brought up. The consumer interest in medicine is always a bad idea because the uninformed consumer is gullible. The largest fraud in Medicaid system is due to fraud on the provider ( read dr and clinics) not the patients.

This guy has absolutely zero knowledge about Singapore healthcare system. I’m from Singapore, in Singapore as long as you work you pay a government insurance premium scheme and you can use it for you or your dependents. It’s not Medicare for all. That’s not the end of the story every medical procedure you do and every hospital stay is capped like maximum hospital stay is capped at $5000 per year and maximum per day is capped at $250. This hardly covers even 50% cost of any major procedures. Singapore also allows employers based private insurance which let you go to expensive private hospitals and these insurance companies normally massively profiteering from young and healthy people also creating a class difference between government insurance and private insurance. I don’t know a single Singaporean who doesn’t complain about their healthcare cost. This is classic misinformation spreading.

David Tilli Of course the guy who’s pretending to be expert on Singapore healthcare. For past 15 years a lot of Singaporeans want European style universal healthcare having said that Singapore government insurance is lot cheaper than US private insurance but it hardly covers major medical procedures due to capping on almost everything.

As a Canadian, I find this debate a tad rich. We managed to build an effective, working single-payer system, with a tax base one-tenth that of the USA. It ain't perfect, but Canadians don't have to risk bankruptcy over a serious illness or injury.

@@smartestguyintheroom6341 medical care is not rationed at all, zero, nada, if 10 thousand Canadians get cancer or a common cold, then 10 thousand get treatment, real no holds barred treatment. You seem to think the Feds dictate medical procedure, they do not. In Canada all medicine in private, private hospitals, private doctors private scientists, the only thing the feds do is act as a giant insurance company, they negotiate every few years with private medical associations of costs of procedures and presto that's it, nothing more. No gov't dictates or medical interference, just payments to doctors only. It's fuckin awesome, way better than your 5 million rules and paperwork and endless money system.

All governments spend less than the US. That's not unique to Singapore. I get free healthcare in my country. Healthcare isn't like buying cereal. No one goes into hospital or takes drugs because it's free. It's not fun like eating cereal. Not to mention that's not how it works. You need to have a doctor approve things and in a socialised system they don't want to waste money. In private care doctors are more likely to perform unnecessary procedures etc as it's the patients' money and the doctors make money from it.

The entire debate boils down to that part around 10 minutes, when Sarwark says that making a system that works for 80-90% of people is good enough and Seder points out that the 20% who'd be left out are the only ones costing us anything in the first place. And the further they elaborated on that, the more Sarwark engaged the notion that just not providing care for those people was the way to go. Not on a state level...just them not being able to access any care at all, because they wouldn't be able to afford it at a price at which firms could profit. I really wanted Sam to press in on that point, because I'd like to know what Sarwark would expect to happen in that scenario, and whether or not he'd be comfortable with the logical conclusion of 20% of our population dying a miserable death and probably exposing the rest of us to a host of diseases, wreaking havoc on the economy and the social structure of the country, etc. Of course, that's not what would happen...the poorest with unmet needs would die off, and the ones who could pay to leave for a place with better access would and would drain their resources instead of ours. And he'd be fine with that.

They would spend 1Million$ to help 1,000 people but dont dare ask them them to spend 2Million$ for 1,001 people. As long as you are healthy enough to not cause cappies to spend *too much* money, then it makes sense to be covered. But if you have a terrible disease, then theyd want you to suffer and die bc they would hate to think someones life is worth more than a certain amount of money that they deem could be used for a private service. I pay $150 biweekly for health insurance for me and my wife. And it is ABSOLUTE GARBAGE. I would rather give that money to the government plus more for a "Medicare for all" system.

Spend a little more per month in Canada and I've never had any trouble with clinics or hospitals. No copays, no deductibles and the premium just kind of comes out of my taxes, I never see a bill. Had a bad car accident a few years back, quite a bit of in and out of hospitals and rehab, no bills.

I'm from Australia, and it's like this in my big dumb country: I pay a levy in my tax each year I have a card with my name on it I go to a dr when I'm sick It is free at service If I want a premium service, I could get a private insurer, but that is a choice. It's not perfect, but at least our insulin won't bankrupt us. Anyone arguing against M4A is Lightspeed stupid or hideously dishonest

Let me preface this by saying I acknowledge the problems the VA has had. However, as a veteran that has largely received excellent VA care that has matched or exceeded my experience with private care (in fact I've seen much worse in private hospitals), it really irks me to see libertarians/conservatives knee-jerk dismissal of the hard work put into a by-and-large efficient and reliably run organization that makes a difference. They've heard the worst case scenarios parroted, over and over, by political operatives and talking heads, so much that they seem to believe most veterans covered never even get to see a doctor. It's another example of how conservative propaganda has disconnected them with reality.

In a Libertarian Utopia, all Emergency Rooms have WiFI (not free of course) so that patients can search the definitely-not-gamed reviews about how to best treat their internal bleeding, and whether they should walk to the other hospital 10 miles away because the surgeon on staff right now is only a 3.2 on 5.

You can't just read economics 101, and believe that you know everything, like how all libertarians act. Healthcare can't be compared to the demand for any other normal goods such as groceries, because demand for healthcare is inelastic. So profit maximization in this case does not ensure that people get the best possible service at the least cost.

What right libertarians need to understand is that there is no such thing as free market healthcare. And I don't mean "We've never seen free market healthcare implemented". I mean it doesn't exist at a conceptual level. The free market relies on both parties of an exchange working with an equivalent leverage, access to information, and ability to say no to the transaction. It relies on the consumer having the ability to perform a comparison of both price and of quality of product. This does not exist in healthcare. You do not have the ability to walk away from a transaction when a company owns the patent of a medication you need to live. There was a viral video a few years ago of some guy perfectly demonstrating this. His wife was pregnant so he called around to nearby hospitals to try and get quotes or estimates on what it would cost to have her give birth there, and the response from hospitals was either "We don't know until you come in and actually get the service" or "We won't tell you until you actually come in and get the service." If i was trying to buy a car and the car dealer told me "You won't know the price until you sign on the dotted line." I would walk away and go to another car dealership. But you can't do that with hospitals, especially when every single one basically says the same thing.

I generally agree with you (as someone who opposes medicare for all) but my biggest issue is medical R&D and private investment. If the USA could adopt a more German or Australian style system or maybe only socialize certain aspects of the healthcare, that would be more acceptable. I really want to avoid having the government manage the entire system and I would prefer something more decentralized as well as some sort of profit motive and competition in the right places.

@@cauyawolfe4724 Okay. I asked because your comment really doesn’t seem to make much sense, nor does it seem to address anything I said. “Free market healthcare” doesn’t exist. It, as a concept, is only a fantasy. So doing things like having doctors have “procedure menus” and other pro-transparency measures-while definitely a good idea in and of themselves-won’t work due to the power imbalance between patient and industry. When Martin Shkreli can raise the price of a drug by hundreds of dollars per pill and your only choice is to pay or die, it’s obvious that transparency isn’t the issue. You need to just decommodify healthcare. Any attempts you make to improve the private healthcare market will always be inherently inferior. Some things should not have a profit motive. Also, pharmaceutical and health insurance companies are not tax-exempt to my knowledge, and are quite openly for-profit companies. Many are publicly traded.

@@cauyawolfe4724 You don’t die if you don’t buy a car. You die if you don’t pay for insulin every month. A deregulated market would just boost the ability of drug manufacturers to jack up prices. Mandatory price caps would be an immediate and efficient way of lowering drug prices. Far better than getting the government out and hoping that the market just incentivized drug manufacturers to lower their prices rather than just form a price cartel to keep prices and profits high. Other developed nations have universal healthcare. There’s no reason to try to make things so difficult when the rest of the developed world has a super easy way of handling healthcare.

This should be obvious, but laser eye surgery is elective and usually not an urgent thing so you can shop around. You can’t take your time to shop around for the best ambulance service or the best hospital when you are on the ground having a heart attack or when you are unconscious from a concussion.

Incorrect. Profit implies that two or more parties benefit from an exchange. If you believe that profit is inherently bad, then you've read too much Marx. The labor theory of surplus value is a stupid idea. Start with "Human Action" by von Mises, and go from there.

The worker will starve to death and is negotiating at the barrel of a gun. This built-in imbalance means the worker must capitulate sooner than the capitalist, and can not spend as much time and effort ensuring the deal is fair. This billions of times over leads to vast injustice.

Sam nailed him when at the end he clearly told him that they both came to the conclusion that healthcare if applied properly to do what it's intended which is take care of people, therefore cannot exist as a profit based system because it's a contradiction and will always fail. Hence why it must be a a non profit system which because its non profit it cannot be run by the free market which run on the principle of profit first. So by definition we must give that up if we accept to live by libertarian principles (aka the free market can solve it). Therefore libertarianism makes no sence, its like saying I wanna feel more freedom therefore I will cut off both legs, and the rest of us say no thanks I rather stay here and be whole.

How do you think profit is achieved? By satisfying a want/need. The basis of capitalism is using supply to meet demand by serving your fellow man. Profit implies mutually beneficial exchanges between two parties, and it is natural for capitalism to be the engine behind healthcare. Resources are scarce, which means that scarcity implies competing ends. Unlimited wants, but limited resources so society must decide how to optimally allocate the resources to advance the betterment of society. If you take away the profit incentive, then there is no room for improvement, innovation, or quality. Governments are susceptible to the economic calculation problem for this reason.

I had physical therapy last year... they charge you if you get heat applied to the hurt muscle. They charged me 130 dollars to use a heating pad for 10 minutes. The problem with the hc system is greed imo.

Absolutely. My mother for years has put a little money in a checking account for emergencies and given me an ATM card to access it. Not once even in emergencies have I used it. That's my own mother. If my neighbor were to do the same thing I would be even less inclined to spend his/her money. Libertarians don't give a shit about their impact on other people so they assume that everyone is like that.

So I got a broken arm and now I have to price compare which hospital has the better prices before I go for treatment? What if one hospital has the better reviews, but the price at another hospital 10 miles away has the better price for Xrays. Now me not being a doctor how sm I suppose to know what other line items I need for my treatment? What if I need an ambulance? Now while I'm in pain, I'm supposed to have the frame of mine to ask what the pricing structure is for the ambulance? The point of single payer is to make any already vulnerable situation less difficult. You are sick or injure the last thing you should be dealing with is cost. You as a human being should be able to get care no matter what your financial situation is for what ever necessary procedure you need and not find yourself in debt because you had the misfortune of getting sick or injured. That is not s strawman agreement that is a fact. Healthcare is not commerce, healthcare is a basic human right. If you don't understand that then the fault is in you.

You should be able to defend your position with honest arguments, and not reflexively argue against everything. You got a minor issue that every doctor does OK, you go for price, you got a thing where the quality varies greatly, you go for quality. You break your arm, you go to the closest hospital and get it taken care of, it is emergency care. Then you add ambulance in your company to. Sorry, he is talking about non-emergency healthcare. Amblance is free too, they are not taxis sent to evabody and they mama going to hospital, only used for emergencies. If you have a broken arm or sprained ankle, you must call a cab. You have to make up non-reality scenarios to argue your case, cannot be much of a case you have then. You strawmanned him and then me, that is a fact. And you strawman now again, while trying to escape the fact that you did strawman. Making it out to me being against healthcare for all, is a strawman. I never said so in my previous comment. Healthcare is both a public service and commerce, in all countries. It is not a human right. The right to choose though, that is a human right. I've seen a lot of anarchists at least acknowledge that there would need to be a massive paradigm shift in society towards more social responsibility for caring for the less fortunate and things like that. They seem to acknowledge that just flipping the switch to a system like that could never work without massive societal belief changes and behavioral shifts. I've never really heard a libertarian argue with any passion for anyone less fortunate than them to have anything, unless it came as a side effect of them not having to be bothered by anyone or anything.

Do you sometimes imagine what an alternate America would look like where we have a Single-Payer Healthcare system like the rest of the world, but we have debates on whether or not we should socialize fire departments? "I just think that it takes away American's freedom to not allow them the choice to barter for if they really need a ladder to put out the fire in their house. I don't have a very tall house and the fact that I would lose money in my taxes to have to pay for a ladder is absurd for me. Furthermore, I don't see why we need to get rid of Extinguisher's Insurance to help pay for Fire Fighters. I love my rates and the copays aren't that bad."

We, in Canada, don’t understand the American’s objections to single payor health care. In the mid tax bracket, for example, I pay 26%of my income in deductions. They include federal & provincial taxes (which includes 100% healthcare), CPP (pension) & UIC (unemployment insurance). So, I make $50,000/year ($4,166/month), my take home is $3,083. A livable amount. My health care includes pre-existing conditions (neo-natal to death), doctor & hospital visits, surgery, therapy, etc. There are no co-pays or deductibles. There is also no paperwork. It does not include elective procedures like face lifts. Private employer or private insurance covers vision, dental, medication and many other medical assistance such physio therapy & chiropractic. Our drug prices are 10% of the US. We receive 1 year parental leave. Figure out what you pay in taxes & health care. Is it comparable? So, my question to all Americans, what do you have to lose?

Most progressives are just uniformed on healthcare. No American goes without healthcare in the USA. County hospitals are free. Poor people can get Medicaid. Low income people can apply for reduced rates at all hospitals. Religous organizations will help pay your hospital bills, ect. We can do all this WITHOUT a ridiculous tax rate now.

In our current insurance driven system, patients are not conscious of cost. I took my daughter to the hospital when she got hurt when she was a toddler, and it cost $10,000 to keep her overnight. They literally charged us $100 for every kleenex tissue used while she was in the hospital. And it didn't matter, because the insurance company was paying for it. And even if I wanted to, there was nothing I could do about it. My daughter needed treatment. It's not like they asked me if it was ok for them to open a box of tissue and charge me $100. I had literally no choice. If I had declined treatment, I would have been arrested for negligence in not taking care of my daughter's healthcare needs. This guy's argument about the consumer needing to care about cost makes no sense in the context of our current system. And while over-paying for tissues is a huge problem - do we really want to be concerning ourselves with cost when it comes to the care that we receive? Did he not hear about the guy who recently died because he took a no-name brand of insulin so that he could save money for his wedding? That's the kind of tragedy that happens when you make the consumer of healthcare be concerned about pinching pennies. Life saving medicine is not the place we should be worrying about the cost. The problems that needs to be addressed are 1 - how do we pay for vital healthcare that people cannot afford, and 2 - how do we keep the health insurance industry from fleecing us and charging $100 for a tissue that should cost $0.01, or worse, $1200 a month for insulin that should cost $9. That kind of price gouging on services that are necessary to life is illegal in any civilized society.

The way we do it is with a monopsony, which means a system when there is 1 buyer, i.e., single payer healthcare. If we all pay taxes to cover healthcare and the government is the one entity paying hospitals for their services, then the government can dictate the price. As long as the government does this well - and the government is very capable of doing stuff well in many cases (I'm being sincere) - then we would get lower prices than we could in a market and we'd achieve 100% coverage which is impossible within a market

Actually, I think that state provision - as in our NHS in the uk where I am, but I am sure elsewhere - likely DOES impact behaviour. We have the sense that need should be treated, irrespective of income, and that we have a collective sense that everyone belongs to a community that acts together, for each one of us, and not as a mass of atomised individuals who must look out for themselves. It's this our Conservative government is trying to erode.

You should make your own decision. How do I make the decision? Speak to your doctor. So, a doctor recommends the best treatment is X surgery. X surgery is the most expensive, so I should ask what the cheap surgery is, even though it’s not the specific surgery or procedure I need? I’m not a fucking doctor dude. And healthcare shouldn’t be primarily treated like a commodity.

The thing I hate about the lasik argument that ppl rarely pt out is the fact that cheap alternatives exist. Very Rarely if ever do people NEED lasik. Glasses and Contacts exist while this is not the case for many illnesses. Its not like with cancer there is a cheap alternative to Chemo. When you need a heart bypass theres no substitute for that. When you need hip replacement theres not alternative other than downing painkillers to save you from the joint pain. Libertarians always talk about lasik eye surgery but completely ignore that its optional and has tons of cheap substitutes to doing it.
