I think it depends on activity and the plane in which you are weak.

This is awesome! Since there appear to be some immediate (although not long-term) activation changes in scapular movement after scapular training, there may be some merit to doing motor control exercise as a warm-up to more conventional shoulder strengthening rehab movements where that motor control can carry over to exercises with more concrete utility. It'd be interesting to see some research on scapular motor training in that context!

So the take away from this is that scapular dyskinesis is not a problem and that normal strength exercises of the rhomboids, serratus anterior, rotator cuff and trapezius are good enough for the pain in the shoulder to be reduced?

Wow Great Stuff guys, really makes alot of sense. Scapular exercises are difficult for the patience and im sure alot of them do not adhere to them beyond the treatment rooms. i have found better outcomes with just sticking to lower and med traps strengthening

We would go as far as to just exercise the muscles that attach to the scapula. The upper traps have been falsely demonized in the past. Have a look at our upper traps tightness video for more details

There is dsykenisis but it looks like this: adduktion and downward rotation of the scapula at the very beginning of shoulder abduction It is pretty rare though Ask PTs if the Lower Traps are scapula upward or downward rotators... The neumann kinesiology has a great chapter about it The worst about this topic is the down-and-back cue that depresses the shoulder girdle right on the brachial plexus and narrows the subacromial space

Yes, the "down-and-back" cue is horrible and has to be eliminated.

I think my scapular dyskenisis is a functional adaptation. I didn't even know I had it until a massage therapist pointed it out to me! The therapist suggested some exercises and those caused pain. This therapist then suggested I get specific exercises from a posture specialist what with my alignment not being perfect. Again when I tried those exercises all they did was aggravate and hugely exacerbate my chronic pain from PHT and GT. So it seems to me my misalignment (which is not noticeable to anyone who knows me) probably developed as a functional adaptation to my PHT and GT. Perhaps the scapular dyskinesis is part of the adaptation to my PHT and GT which I can't get rid of. Whatever I do, it just makes those two issues worse. Oh well.

Also, serratus anterior plays a big role in scapular movement. so he also must be strengthening. In generally, lower part trapezius and SA are "big guys" in solving this pathology.

Agree, but training those muscles them doesn't necessarily mean that we can alter the kinematics of the scapula.

@@Physiotutors Depends whats the cause of altering kinematics, if it is some injury and those muscle don't work properly because of that injury, training them can get back right function of scapula. But if there is neurological cause of winging then its more complex problem then just working on those muscles

What’s your thought on the glenoid not beeing horizontal enough in endrange anteflexion/abduction because of to little scapulathoracic movement (to much held in protraction/internal rotation) and thus give more chance to cause a subluxation/dislocation? (Ludewig & Reynolds, 2009; Struyf et al., 2011; Kibler et al., 2013; Kibler & Sciascia 2016)

Not familiar with that concept. All we know is that it does not seem to be a risk factor for shoulder pain in the normal people

i have scapular diskynesis also , my scapula makes cracking sounds , when i overuse my scapula hurts a bit , lost some strength , its a little bit uneven. can i train in the gym if i have scapular diskynesis ? my therapist said that i can train but not for too long and no heavyweights. but i want to hear it from you also

Whenever i do the exercise for Scapular Dyskinesis (from another video) i get severe fatigue/tiredness after accompanied with pain in the side of my neck and down on the inside of my scapulae? THis is something ive been struggling with for 2 years, and i cant even train anymore properly. Any ideas why this happens to me? I did the exercise at 120 degrees

Impossible to say from afar. If an exercise is not tolerable, try to modify it or make it easier. If not, don't get to fixated on one exercise, there are many that train the shoulder region.

Is it possible head forward posture is also an adaptation to rather than a cause of neck pain?

Good question. Would have to compare prospective studies to cross-sectional studies. we would also expect that forward head posture "normalizes" as soon as the pain is gone, but don't think that's the case often

Sorry. My oral English level is too low. So what is the idea ? Are exercises worth it to fight against scapular dyskinesia or is there nothing that can bo done ? Mine is severe since my accident 2 years ago. And noone is able to tell me if it is due to nerve damage, serratus anterior, rhomboid or trapezus that may be torn or because two of my rotator cuff tendons are torn.

In most people scapula dyskinesis might be a functional adaptation and it's hard to say what is normal and what is not. Very hard to answer in your particular case. In general the literature is mixed at best to say that kinematics of the scapula can be changed through exercises. It's probably better to focus on general strengthening.

yeah pretty much… it seems most anatomic abnormalities are just variations of normal anatomy. There’s limited evidence for the abnormalities to be pathologic… Basically, if something’s not hurting or limiting, you’re probably fine…

A lot more schooling, a different perspective of anatomy from seeing and dissecting human cadavers, passing an intensive board exam, a focus on functional movement, and a completely different mindset. PT is more focused on helping restore function, is very specific, and patient-centered. More medical focused than a personal trainer and requires more medical knowledge.

I had a traumatic labral tear and had it surgically repaired a month ago. Prior to surgery and starting about 2 weeks post op, I started having scapular pain and a sensation of something catching. Since my surgery, the scapula on the side of the surgery does not move much. It doesn't wing at all. I'm having significant pain in the scapula and my lower back. My therapist believes there is inflamed bursa under the scapula. Hopefully this heals because I am a nurse and need my shoulder function for my career.

Hi Jessica, I am sorry to hear that. I was wondering if you could give me an update on how you were doing because I also have been diagnosed with a possible labral tear and believe I have scapular dyskinesia. So I am a little worried if the surgery will fix the issue. My surgeon himself is not entirely sure, thus he is conducting a diagnostic procedure to first confirm.
