Anesthesiologist, Dr. Zach Antonov, joins the Glaucomfleckens to discuss their shared background in comedy, answers the burning question of if anesthesiologists mess with patients while they are under, what fears most patients have, and the cocktails it takes to keep people asleep and then wake them up.

I’m a natural redhead, anesthesiologists have fun with me and I’ve had a LOT of surgeries. Remember hearing the anesthesiologist during my first knee surgery going „Wait! Wait! Don’t cut! She’s not out yet!“
@sanachanto
@sanachanto 11 месяцев назад
I’m a natural redhead too, although more of an auburn as I’ve gotten older and less ginger. I’ve been aware that some of the genes that cause red hair can make anesthesia less effective for us (and interestingly make opioids more effective), but I’ve only ever had local anesthetic or IV sedation for dental work, which I know is different from general anesthesia, and so far seems like it doesn’t apply to me. I’m not itching for a major surgery but I’m really curious to know for certain! But I’ve missed out on some other common redhead traits like easily burning and freckles, so I won’t be surprised if I’ve missed out on this one too.
@katwitanruna
@katwitanruna 11 месяцев назад
@@sanachanto I’m pretty sure I saw something about them finding the gene that fubars reactions to anesthesia although I have to say that personally I barely react to opioids. When I had my first two full knee surgeries they had to give me two mepergan every four. And I was off of them before my surgeon even talked about dropping down. Heh.
@lmboh8585
@lmboh8585 11 месяцев назад
As a redhead, I find that anesthesia and opioids hit me fast, but then wear off just as quickly. I didn't understand it until a dermatologist who was doing a bit of MOHS surgery on my forehead had to keep adding more deadening medicine to my head (I forget what he was using, it was long ago) because I told him that I could feel what he was doing. He finally asked me if I was a natural redhead, I said I was. A lightbulb went on for him and he explained the studies about redhead genes and anesthesia/opioids. BTW, @katskatsnews, if I had heard the anesthesiologist say that, I would have been frantically trying to call off the surgery! LOL! I hope your knee is well now, I just had a replacement 6 weeks ago and it went very well.
@katwitanruna

I didn’t like the commercials on the radio during one of my surgeries and when I went to complain heard “DON’T MOVE! WE’RE INSIDE YOUR LEG!!” and that’s the day I found out I learned I’m a “rapid metabolizer” 😂 downside, it only got more traumatic from there, when I started realizing I could feel all of it 😅

I work in a peads ED and whenever the RMOs and Regs rotate the nurses get a briefing from our nursing director to remind us that the doctors are new, most came from adults and to double check with the consultants if they do something odd like order a CT instead of an MRI. So not only are the consultants watching but the nurses are too

When you are in labor that anesthesiologist is an ANGEL from HEAVEN. Epidurals can go wrong, so a good anesthesiologist is worth their weight in gold. I was the same, went from mooing through contractions to napping within minutes. I went into labor at 2AM so after a few hours I was tired just from not getting enough sleep. I napped for a few hours then I popped that little guy out with zero pain.

I once needed an IV in ER. There was a new nurse and she struggled to put me the IV. She apologized and the veteran nurse did the same. My answer was that it was ok, I was once a newbie in my profession and I know what is like. They fixed my problem and I was happy as a patient that this helped her to become a better nurse in the future.

Humor is definitely good medicine. Couple of years ago I was in hospital for an infection and getting every test under the sun. When the pathologist handed me the bag for a stool sample I just looked at her and said "Well, this is a pretty shitty situation to be in now isn't it?" She looked at me dumbfounded then cracked up, I finally broke her, ha!

I had a jerk of a surgeon who stomped out of the OR and yelled all the way down the hallway to pre-op about how he's ready to go and anesthesia is wasting his time. When he saw that I was awake, then heard that I was on my seventh try to get an IV, he turned on his bedside manner, gushed a hello to me, and hovered impatiently. I have to wonder how much time HE wasted by not going back to scrub when he saw that anesthesia was working on it! 😂

I just graduated as a pharmacist and my one notable interaction with anesthesia on rotations was them telling me they didn't know what happened to an unaccounted for, unused but definitely pulled 100 micrograms of fentanyl. I wish I didn't think of this every time the profession was brought up but like, it was probably the scariest thing I saw on rotations.

this was lovely. had a c section and the anesthesia team was absolutely great at talking me through the case (that's what I wanted, it gives me the illusion of control). I was fascinated because it was not at all my first surgery, but the first surgery where I stayed awake and met all the team. Even the surgeon talked to me! And to my husband, to tell him to stop making me laugh. But there was a big Glaucomflecken moment when anesthesiologist number 1 tapped out and introduced his replacement halfway through the case. Made me grin.

So Dr G - do you ONLY do cataract surgeries - no like strabismis, retina, eye injuries? Only cataracts? It would be nice to hear the whole scope of what you do in your practice. And, most people having cataract surgery are terrified coming in just thinking about it. Or even the decisions that need to be made - regular lens, toric, etc... Or, like in my case I requested one eye to be for near vision and one for far (I always only focused one eye at a time). I was put out for the initial shots just momentarily. It would be nice to have another doc interview you on all the ins/outs of cataract surgery and options. What about failures - are there any? Do you ever have to do the surgery more than once? Do they fail over time? Mine cloud up like a dirty contact sometimes - why does that happen. See.... so many questions. Don't downplay your profession.

When I had arthrodesis surgery on my foot, the plan was for nerve block so the rest of the anesthesia could be “light.” After the nerve block & propofol, the surgeon stroked the bottom of both feet. When he did the non-blocked foot, I apparently pulled both knees up to my chest & they couldn’t straighten me out. Being a flight RN, I had previously requested rocuronium over sux so I didn’t get the muscular aches. When I woke up, he said I guess it is a good thing you brought that up

I had a general anesthetic for my scleral buckle. I told the doctor that there was no way I was going to be able to handle just the nerve block and being awake, so they hooked me up with some propofol and that was all she wrote. edit - worth noting that I had taken 2mg of alprazolam upon learning of my slowly detaching retina and it didn't even take the edge off. No way was conscious sedation the way for me at that time.

Phlebotomists always get my vein first try, but nurses have to try three or four times in each arm until they give up and do my hand instead.
@celestialnurse07
@celestialnurse07 11 месяцев назад
Because they do not have to leave a device inside your arm like we do. Try leaving a straw like device inside your vein and have it stay put and hopefully the end of it not sitting on a vein valve or not puncture thru or the vein blow once we start pushing fluids in then tell me all about it. As soon as phlebotomists see blood return on their device they draw blood and get out. Us we have to do a lot more.
@Silkenray
@Silkenray 11 месяцев назад
I don’t just mean for IV meds. Nurses struggle to find a vein on me to fill up 2-3 vials for a blood test using the same equipment used by the phlebotomists. It takes 4-5 tries. Please don’t interpret this as me trying to badmouth nurses. You are overworked and underpaid, and are essential to every aspect of patient care. Which is kind of the point. My working hypothesis is that since you have to manage so many different things with a high degree of proficiency, that in the specific area of drawing blood a phlebotomist who does it continually all day every day stands a better chance at sticking difficult arms like mine.
