Often, yes! The SSA/ DDS often sends on this form "{Work HIstory}" - well before even making the decision [whether you do -or do not- "meet or equal a listing" . They may even send you the "Adult-Function Report, as well" !

Come with your Current state Driver's license/state ID card and if possible, your SSN card, and the rep can either: 1) provide you with this info in person; or assist you in ordering this info from the SSA/IRS. or: 2) Direct you to the IRS to fill out form(s) to order this info/ copy.
