On friday, the Indian Film industry lost another of its young and bright stars to an unexpected cardiac arrest. Puneeth Raj Kumar was only 46 years old, was a healthy fitness enthusiast and had just finished at the gym and then started complaining of intense fatigue. Unfortunately his heart then went into a very fast but ineffective rhythm called ventricular fibrillation. As no blood was being circulated by the heart, all the organs started suffocating including his brain and by the time he reached hospital it was too late and he was pronounced dead. He leaves behind a young wife and two daughters. Very much in keeping with kind of human being he was, he had asked for his eyes to be donated in the event of his death and his brave family made sure that his wishes were respected. This is a devastating story and all we can do is wish for stength for his family to bear such a huge loss.
I wanted to do a video on sudden cardiac arrest and sudden death. I can understand that some may not want to watch this due to its content, it does have some messages that may be helpful. I dedicate this video to Puneeth and those that have been left bereft.
Sudden cardiac arrest refers to the sudden cessation of cardiac activity leading to no blood being circulated around the body. This is usually due to the heart going into an abnormal heart rhythm which results in the heart not beating at all or the heart beating so fast that it is completely ineffective. The latter is the most common heart rhythm disturbance and referred to as ventricular fibrillation. If cardiac activity is not restored in some way then inevitably this leads to death which is then termed sudden cardiac death.
Sudden cardiac death is not uncommon. It is estimated that 15% of total mortality in the western world can be attributed to sudden cardiac death. The risk of experiencing sudden cardiac arrest can be increased by a number of factors but the most important 2 are:
Increasing age
Underlying cardiac disease (which may be congenital or acquired).
In general sudden cardiac arrest is seen 2-3x more commonly in men compared to women. If there is a history of clinically recognised heart disease (congenital heart disease, angina, previous heart attacks, cardiomyopathy and heart failure) then there is 6-10 fold increased risk of SCD. In addition if patients dont have known heart disease but have major risk factors for heart disease then the risk is increased by 2-4x. SCA is the mechanism of death in 60% of patients with known coronary heart disease and it is the initial clinical manifestation in approximately 15% of patients.
What are the causes of sudden cardiac arrest:

Doctor, if you are not already a lecturer to medical students, then I implore you, as of today, to realize the enormity of your communications skills. I would literally love you to repeat, word for word, the talk you gave here in the lecture hall for students of heart problems... they would learn more from you in 20 minutes than they would in hours reading a textbook, or from some god-like superior heart surgeon who has contempt for those who do not know all he knows... You are very upset by the loss of this fine actor.... so I send you my condolences, as well as to his family and loved ones.

I agree Dr Sanjay would be very beneficial to students . His knowledge is vast but it’s his approach, showing compassion and humanity and caring for people is what makes him different . The students need to learn that . The arrogance, being unapproachable, lack of compassion has to go from our future doctors . Also , As a society we accept that in other professions some people are not good at what they do .For example you can call a plumber and then criticise him as he’s not competent . But when it comes to doctors we tend to think/ accept that they all are good at what they do and they ALL have good knowledge . But of course that’s not true ! . Many doctors don’t like it when you ask them questions because they don’t have the answers .

And was I was told in hospital that I now have Atrial Fibrillation with Arythmia . As a child I suffered from Rheumatic Fever directly following immunization against Polio. I am from South India neighbouring state of Karnataka. I have been following the news, I have never seen so many people devastated by the loss, not because he is(Still alive in people's heart) a popular actor but he is one of the greatest human beings and down to earth person. Many popular actors across India paid tribute and they were all talking about his humanity and personality. I have never seen such a respected human being in my life. And, I have never seen a news reader crying like that by this news on live TV.

This is a caring down to Earth doctor who is educated and intelligent and actually really cares about explaining these concepts in his videos and cares about the audience and fans who watch his videos. The best of RU-vid to learn this stuff. Look no further than this guy.

Thank you for your excellent presentation. My heart breaks for his family. 20 years ago I was required to take a CPR class. I was able to administer CPR until emergency arrived. Learn CPR! ❤️

I was shattered to hear the news, and trust me even people who haven't watched his movies before actually cried when they came to know about it because he was a very nice and genuine human being. Exactly 24 hours ago almost to the minute, I was on the phone with my sweet sister having a wonderful conversation when she abruptly dropped the phone. I could hear her son and daughter in law say “she’s not breathing do CPR” I felt so helpless. All I could do was hit my knees and pray. I stayed on the phone until I heard the EMTs arrive. By the time we made it to the ER, she was pronounced dead. I am so beyond heartbroken. I feel shattered. She was mid sentence and just dropped. I cannot believe this. God bless you and so sorry for your loss.

All of your presentations - Thank you for sharing your knowledge and insights with "accessible" thorough explanations, stories and personal experience. Your delivery, from multiple perspectives, is received as sincere down-to-earth compassion. I really appreciate your final thoughts expressed at the end of this presentation - One approach to living (medicine too?) is not just maintaining the machine or extending life ... It is also pursuing a fulfilling and sharing life. If one chooses so, uncertainty can be a reason for motivation rather than a source of fear; ... More living-focused than survival-focused.

I have this condition. My heart went into abnormal arrhythmia March 2020, I have a ICD implanted and it saved my life. My heartbeat reached 395 bpm I passed out in seconds and my ICD discharged five times. The first shock saved my life the other four shocks were “bonus” shocks (continued to shock until heart rate dropped below 180 bpm). After this event I developed panic attacks and officially been diagnosed with PTSD which happens with ICD patients. One of my cardiologist said without the ICD my chance of surviving this event was 5 - 10%.

I can't imagine surviving an event like that without residual trauma. I am glad the device worked and I'm terribly sorry you suffered a trauma response. If you haven't already, seek out a therapist who specializes in PTSD and give EMDR a try. It's known to be very effect for helping with events like this. Be well.

@@lin1681 When I was in my 20’s I tried to join a hospital gym, before joining members had to have an EKG while walking on a treadmill. They hook me up and wouldn’t let me walk on the treadmill they told me I have to see a cardiologist. My first cardiologist did a Holter monitor and the report came back I had 24,000 extra beats in a 24 hour period. He also told me I had a condition called “sudden cardiac death” and said not to worry until I started passing out. With the many extra beats my heart muscle has enlarged so officially have Cardiomyopathy/heart failure with runs or episodes of ventricular fibrillation. In 2006 They implanted an Pacemaker/ICD (Internal Cardioverter Defibrillator) Anxiety can throw me into a serious arrhythmia, in 2007 I was speaking at a ministry meeting got nervous and it shocked me three times. In March 2020 I was having a normal morning getting ready for work and without warning my heart glitched, I passed out, came to on the shower floor with my defibrillator going off with the first shock at 6.9 seconds. I told my husband if I would have died it would have been peaceful. I made it into my 60’s and Thank you! I am glad I am still here too! If you have concerns with your heart go see a cardiologist and have them run some test.

Dr. Gupta. I recently stumbled upon your videos and am learning from them. I am a cardiac nurse and will never understand everything cardiology- wise. I, myself was told I had SVT but after wearing so many monitors for long periods of time, it was never captured. I also have sinus tach occasionally up to 120 on exertion. Don't feel the cardiologists are truly trying to figure out why my heart is funky. Your videos are comforting (not this one, lol) and you have the best "bedside" manner I have ever seen. Peace to you. Looking forward to your next video! 😇

Hopefully you were mis-diagnosed? Since its never been able to be caught while wearing a monitor? Maybe sinus tach is all it really is?🤔🙏

Hi. I have problems that are similar. I had to fight for 15 years because I had WPW and no one believed me because it started at 19. I finally had it and found a doctor who would go in and check. He cured me. However I still have the issues you have and unfortunately the doctor moved away. I am searching now for a new one. Don't give up. My heart rate jumps to 170 in the morning sometimes. It's crazy. However, there are doctors that will care. I'm surprised you being a cardiac nurse doesn't give you a lead. Anyway, be well.

I lost my husband on the 28 September 2016. He was normal. Collapsed and passed on immediately at 9:30 in the night. I was finding answers before his death on diabetes and hypertension. But it was controlled. After his death we looked for answers. I came across your talk and which makes absolute sense now after Gangly,Shula and Purneet Rajkumar left us . These kinds of death cannot be explained why it happens. My husband was 68. But lost his father at 45. Now I see Purneet's and Shukla's captivating smile and positive energy makes me realize that they were real earth angels. We will never understand our stay on earth. Please take care and be safe. Your talks are informative. Gives answers and peace of mind. Rushen, Trishen and Suminthra Oojugir Kwa Zulu Natal South Africa

Thank you for an informative talk on sudden cardiac arrest and also the lifestyle choices we can make to enhance both our cardiac and overall health. Just as important...your final message on living life well. As a 68 yo, active, healthy ( never hospitalized ) retired surgeon, I have always lived with the knowledge of significant family history of CVD. Recent coronary calcium score has shown a high score ,meaning that despite my lifestyle, and being asymptomatic, I have significant risk. Knowing this inspires me to continue to follow the advice of my health care professionals, but more importantly to do as you advise...continue to live the good life and be mindful of everyone and every minute. Thank you, Dr. Jim Waller
