I just want to go through with you as to you know what to expect if you're going to be coming in here for a UroLift procedure
I've done a lot of UroLift procedures and been designated as center of excellence which just means I've done hundreds of these and have a lot of experience in treating patients, around 65000 men that I've treated for prostate enlargement conditions over the last 25 years so I know a thing or two about it
Let me tell you about what to expect
If you're going to be getting a UroLift procedure what we basically do is before you come - I always have everybody take just a mild sedative a milligram of ativan.
It's just nice because a lot of the discomfort that might occur
it's unusual that anybody has significant pain during this procedure but a lot of the discomfort is from anxiety because you're tensing up and when you tense up you tighten your sphincter
Think about it if probably just me talking about it you're tightening your sphincter and when you
do that during the procedure it can be painful so relaxation is critical i always give men a milligram of ativan to take about an hour before their appointment so that when they come in they're fairly relaxed they're not like falling over but they're just you know chilled and then we have you get undressed of course
And then the next step is to give you a local gel in the urethra okay the gel is chilled okay because it's much more comfortable going in when it's chilled you're getting basically two 10cc syringe fulls of this gel so it's really gonna fill up the entire tube if you will with the numbing gel it's basically a
lidocaine based gel extremely effective
When you put the gel in you want to leave it in for 15 minutes by the clock so those of you who've had cystoscopy and maybe you're going to say to me oh the doctor gave me that gel and it
still hurt when they looked inside my bladder i would challenge you and say they didn't put in two full 10 cc syringe fulls okay so 20 cc's and they didn't let it dwell for 15 minutes because typically you shouldn't have any discomfort after that
There are rare exceptions and i'm happy to discuss that with anyone in terms of what we do in
those rare situations but 95 of the time it's not a problem you put that gel in for the full 15 minutes at that point we then have you position for the procedure so i can get down there with my equipment and frankly we have a little drape going over the chest
It's just frankly again about anxiety the less you see and the less you you know are involved with this the better
So we have a drape set up and you're there's a lovely nurse that's basically right by your head so she's basically monitoring you looking at how you're reacting making sure you're not in discomfort and pain and talking to you and answering any questions or issues may have
So that's really important i think it's really key to have multiple people in the room so we're all kind of a team and we're all paying attention to different things
I'm really focused on your prostate which is where you want me to be the nurse is really focused on your level of consciousness your comfort level just any other issues you may have and we have other personnel that are helping me with equipment it's really important to have a team in place and that's something we take pride in
At this point after the 15-minute dwell time of the gel we then begin you on a gas basically it's a nitrous gas system called products pronox if you want to look that up and basically it's like laughing gas it really helps calm you down
It's simply a little mouthpiece that you're gonna inhale in and out you control it so if it's making you feel light-headed in some way or you just don't like the the way it's making you feel you take that thing off your mouth you breathe fresh air and you feel normal within 10 seconds
It moves into your body and out of your body very quickly at this point we're ready to begin the procedure after five minutes of using the gas your numb and you're maximally relaxed
And i can then take my camera pass the camera through the urethra similar to when you had it probably had a cystoscopy which you really should always have before a UroLift procedure
meaning days or weeks before and once i'm inside the area of that prostate like you can see in this
picture right here i then can deploy the implants and push the loads of your prostate open that portion and usually i'll have to deploy five or six of these implants sometimes seven that portion of the procedure takes about 10 minutes
When we're done i fill up the bladder with water until you tell me you feel like you have to urinate i take out the camera we put your legs back down we take out the gas...

I had the procedure three years ago, with a 60 cm prostate. My doctor put me under general anesthesia for the procedure with a 24-hour catheter afterward. The catheter was uncomfortable...not particularly painful, but uncomfortable, and I went home the same day. For the next week, I had to stay close to the bathroom, because I had very little holding control when the urge to urinate hit (although I never urinated in bed...I actually began sleeping longer right away). No travelling that first week. For about another two weeks I had to make sure I knew where the closest restroom was. It took about two months before I re-learned how to hold myself reliably. But since then, the results have been wonderful, and I'm fully satisfied. My doctor has me come in every six months to check things out, but it's all good.

Just had the procedure yesterday, in the hospital under general anesthesia. It was perfect. I was out for a total of 25 minutes. Anticipatory tension and comfort level was a non-issue. After I woke up, I had a bit of pain for about 30 minutes. The catheter was taken out in Recovery, I urinated and was discharged. I slept well last night and today feel practically 100%. By this morning, all traces of blood in the urine were gone. I was very happy it was NOT done in the office under local anesthesia.

1.5 months out. Usually wake up once but sleep 8 hours about 2 nights a week. Previously always got up 3 times minimum. Doc used fentanyl and propofol. First 4 days following was awful. Now fine and glad I did it.

I had that procedure, the nitous oxide and lidocaine didn't make the poke of needle pain free. The pinch that didn't go away for 6 months until I got tadilafil to shrink my prostrate. If I had to do it again I'm going to try PAE! I sure as he'll won't do REZUM OR TURP!!!

Dr Hyman when did you have this procedure done to you? Also you would have to clobber me over the head cause I don’t want or desire to have something shoved up my private. Rather be knocked out. Oh you failed to mention that there are times when the clips fail, ouch 😮. My friend had the procedure in September 2023 and still having problems in the bathroom. 😮

Dude 5.5 is normal size! I would think a long Johnson would make this procedure more difficult

I had this procedure done in December of 2023 after being talked into it by my Dr. There were so many things wrong I could write a book on this. Bottom line, this procedure did not work. The pain during the procedure- with local was extreme. This procedure should never be done with local and laughing gas! There are so many stories where this procedure was Performed and the clips needed to be fished out using a diff procedure. I just had aquablation where six clips needed to be removed. My advice is get multiple dr opinions.

Urolift completely and permanently destroyed my life !!!! Now I am faced with my new urologist wanting to do another surgery (greenlight) to correct to the botched UROLIFT procedure. I am told I would have to self catheterize everyday possible for the rest of my life. My symptoms now are WORSE than before Urolift. Please be advised and don't say I didn't warn you if you get this procedure !!!!!
