This video is part two of a clinical interview role-play of a counseling client (played by an actress) who is having symptoms of anxiety and is using alcohol. In this section, the client’s educational, vocational, and legal history are collected.

I think it was good that he advised the client when he was switching gears prior to him just asking different questions. I thought it was helpful that he advised her that he would come back to something or would keep it in mind. I liked how he asked her how she thought she did in school and at work and how the anxiety has affected her performance or what she does when she can’t perform. I liked how he tied it all together for the client like by saying how the anxiety has affected her work and education and asked if it was concerning to her.

Heather you summed it up well. Dr. Grande asked questions in a calm nonjudgmental manner that allowed her to really think about how her anxiety affected her.

The assessment process is very in depth and I do like how the client is told that she doesn't have to answer any questions that make her uncomfortable at that time and can revisit. It places little, if any, pressure on the client and empowers her, instead of forcing her to face discomfort early on.

It definitely takes the pressure off of the client! It helps that Dr. Grande reminds the client that they can always come back around to it, if she feels like she wants to talk about an issue down the road.

I agree. Putting her at ease about not having to answer questions that she is not comfortable answering makes her more willing to answer other questions. Forcing an answer would shut her down.

I really like that too, plus it almost highlights that issue for the therapist and you know it's something that may need revisiting in the future.

I agree Rosa, by saying this, the client felt comfortable to openly proceed knowing that she would not be forced to discuss any topic that she was not ready to discuss.

I think it was helpful that Dr. Grande began picking out goals as the client was providing information.

This intake is very extensive and ties to many aspects of the client's life and trying to get all the relevant information not just on the education, vocational and legal history but on the substance use and abuse

I liked that Dr. Grande asked the client when talking about sensitive subjects if she was comfortable discussing. I think this showed how building rapport is the most important aspect of meeting a new client.

I liked that as well! Not only is she a new client, but she is an anxious client. His demeanor and the fact that he attempts to make her comfortable discussing anxiety-provoking/stimulating topics helps establish and build their therapeutic relationship. It appears to make it easier for him to subtly show a relationship between her drinking and her anxiety in these different areas of her life.

Just one component of this role play is the use of goal setting, and in looking over the intake interview sheet, it almost makes sense to talk about goals twice (once after all sections ending with psyc. history, and then again after current life situation). This gives the client time to add, change, or reframe concerns before moving on with care.

I think that's a really good idea I think that if there was that second time or even if the counselor just goes back and asks the question again toward the end I think you would get a more detailed answer.

As a patient, I probably sound really irritable when I go through this, but that’s because I hate talking about myself. I don’t like having anyone look at me. I don’t like attention on me at all. So it’s really hard. Plus, I’ve had to tell my story so many times because I’ve had to switch counselors over the years. It’s hard to know what to say or how much detail to go into. I still don’t feel like I’ve ever gotten to the bottom of it all.
