Exactly as shown. To pull the top of the console off, I removed the bins from the storage area under the armrest and the rubber liner for the cupholders. Once those were clear, I grasped the cupholders and pulled straight up firmly. The console came off and with a little shifting, the label was visible. Poor eyesight notwithstanding, I took a photo of the label to get the number. It is located in the upper right corner, but printed upside down. Thanks to the video!
For that same fee Ford (or your independent mechanic) charges to retrieve your code for you, they will also reprogram it to whatever numbers you want. Like your childhood zip code for example, something you'll never forget.
Thanks! If you have a limited/ambient lighting, there is a plug under there you have to disconnect, it is about 8 inches long.. so don't just go yanking.
