Thank you Dr Ali. The lchf is obviously a “one size (does not) fit all”. Your reesearch and focus will get more of us under safe dietary umbrellas Please don’t fail to include normal/low bmi folk w/ir or existing awterial disease from inflammation/auto-immunity. We need your excellent counsel as much, or more… am so glad you are constantly questioning and adjusting as you learn more❤️👏👏👏

It seems quiet and understated, but this is the most important presentation to the low carb community in a long time. I have listened to this 8 times and implemented with a PSMF diet with dramatic results. BG in the 80s most of the day. Morning gluconeogenesis greatly reduced.

Warning: This post is a long one but its what I have done in the last 7 months to lose over 100 lbs. I did exactly what he just said without knowing what I was doing. That's crazy. Back in July 2021 I weighed 335 pounds. I had already tried fasting prior, but in July I did a 7 day water fast. During those 7 days I researched superfoods and made my shopping list. It came down to Grass fed beef (93/7), salmon, sardines, pasture raised eggs, chick peas, avocados, mushrooms, kale, spinach, red cabbage. That was pretty much what I ate. I eventually cut out the kale and mushrooms because I didn't want to eat things I didnt like. I wouldnt eat breakfast so lunch would be a huge salad of spring mix, onions, green peppers, celery, chick peas and avocado. Then whatever my protein was I would do 8oz of either fish or the beef and the another 8oz for dinner. Dinner was a can of sardines, 2 eggs and 8 oz of whatever protein I was eating that day. Dessert was always a big bowl of air popped popcorn plain (no salt, no butter)My stomach was full and I exercised alot. My "cardio" mostly consisted of walking. Is was getting between 10,000 -18,000 consistently. I always met my goal of 10,000 unless I took a day off which was rare. Somedays i hit 20000 steps. I might have jogged 3 times in the last 8 months. I also started to lift weights again. Also started swimming, hotub, sauna. Buts lots of walking. The weight was melting off. Supplements: multivitamin, vitamin D, Magnesium, Prebiotic, probiotic, B12, bcomplex. With dinner I also had what I called my mash, it was steamed broccoli and cauliflower mashed up with salt pepper and tumeric (pretty good). Then I had to add beano for the gas and bloating. So after six months of shopping every day and pooping out leaves in the toilet I started questioning why my GI system was so messed up. I was very strict, no dairy, no sugar, I was even making my own salad dressing until i started using primal kitchen salad dressing. It was one of the only ones without seed oils and sugar. In November I was around 250 lbs and I noticed I started having these cravings so i started eating one piece of fruit. That turned into eating about 6 peices of fruit in one sitting. I didnt like that feeling of wanting to keep eating. So i figured out that the craving were coming from my body not having enough energy because at this point I was basically doing low carb and low fat. but I was still losing weight. My activity level was getting to high for what I was eating. Even though I was full every meal it was like empty calories with all the veggies. So the last week of December I stopped buying all veggies and fruit including avocados and by the end my salad was red cabbage and onions until I finally switched to high fat, and protein. All meat. I had one day of body emptying semi formed stool and haven't looked back since this week I weighed 214 pounds and have never felt better. No cravings no binging and no GI issues. I also cut out ALL supplements. I eat until I'm comfortably full. Some days I eat once, some days I eat twice. I stopped going to the gym for hours a day. I wake up every morning at 4:30 and do 15 min body weight exercise till exhaustion (alt days between up and lower body) Followed by a 15 yoga routine. I recently bout a pullup bar for my upper body days that I do after work. This is a sustainable routine I can do it pretty much anywhere there is a floor. I haven't been this weight since my freshmen year in high school in 1990. My heaviest weight was 380lbs. This was what worked for me. One thing I can tell you is that if your diet leaves you constantly craving its not sustainable. We have to learn to feed our bodies and not our mouths. If your body doesn't get what it needs it will make you constantly want to eat until it gets what it needs. So if your eating alot of processed food and sugar your body will always be hungry and you will always be eating and the cycle will continue until you look in the mirror one day and realize your almost 400 lbs.

The best most easy to understand explanation of metabolism i have ever seen! I was able to reverse my metabolic issues quitting sugar, starch, grains and most fruit and eat meat/fish/eggs, modest dairy, nuts, seeds and low glycemic vegs and fruit. Within 3 months i lost 30 lbs, off blood presure med, no joint pain. My A1C in November, 3 weeks into my changes was 5.7 and my fasting blood glucose was 96. Now my fasting glucose rubs in the 70s-80s and my basal glucose during the day stays mostly in the 80s. Looking forward to my next blood work in April. Yes i exercise 4-6 days a week walking 3 miles daily 2-3 days a week and doing moderate intervals 2-3 days a week.

Most of the time people start low carb diet to deal with pre-diabetes or overweight. This is when they encounter problems of increasing LDL (most don't know whether it is Large or small), and increase in Triglycerides. From this video I understand that somehow manage it with diet to make the numbers in range. This can be possible with exercise and replacing some of the saturated fats with poly unsaturated fats. Everyone is unique, find your balance by trial and error.

The diet that addresses this problem would be a protein sparing modified fast PSMF. Fuel macros fat and glucose are restricted or eliminated while still eating to meet protein requirements. This is done most days with some days eating more fat like normal. I have this problem big time. I am fat adapted in ketosis with low insulin but still have signs of metabolic symdrome. I started my PSMF last week and see improvement in BG and blood pressure. I expect to see: TG drop well below 100 Loss of my 15 lb of belly fat LDLp pattern B become pattern A CAC to stabilize I should add, 3 years of keto and periodic extended fasting has not resolved my residual metabolic syndrome. Still eating too much fat.

Hmmm. Dr Ali, looks like you have been working on solving my particular problem. I will remain OMAD and try ground beef or steak and eggs for a week or two and see if my morning gluconeogenesis slows down.

Update: I am so exited, after 8 days on PSMF, which is a low carb low fat diet, my waking glucose was 98 peaking at 116. Before it was like 130 waking then peaking 150 or more. You have to be careful with PSMF, unlike carbs, fat is an essential nutrient. You have to eat some on a regular basis.

Thanks for this Dr. Ali, very informative as usual. I've been on a PCSK9 for about 6 months, I have my doubts as to whether I'm going in the right direction, but my cardiologist is saying without it I have no protection. I have two stents and a history of CAD. My triglycerides are always around 75 mg/dl and the best my HDLs have ever been 42 mg/dl. I cannot tolerate statins, but I've been taking Fenofibrate in addition to PCSK9 injections. I've been eating low carb for about three years. I know I'm fighting genetics.

@@nadirmirali Thank you for sending the link about PCSK9 inhibitors. I'm nervous about continued use of this drug. I'm also concerned about diabetes since there is some family history that exists. Not sure how to approach my cardiologist, as he seems to believe in PCKS9 inhibitors. I look forward for any future videos you may have on this.

Dear Dr. Ali My case, I am a 48 year old male, athlete, I do intermittent fasting 18:6 every day of the week (with some exceptions) and my diet is low carb, eating mostly nuts, fish (salmon), avocados, salads, chicken, eggs and dairy (cheese). My BMI is 23.5, my body fat is 13% and my visceral fat is excellent according to impedecia meters. In my last blood test (the previous one was 2 years ago and I was not fasting), my total cholesterol was 290 mg/dL (they did not differentiate between HDL, LDL and VLDL) and triglycerides at 180 mg/dL. I feel better than ever, energetically. I play beach volleyball and I have a lot of energy. In the analysis I observed that the Acetone in urine was 10mg/dL so I was in ketosis since I had been fasting for 20 hours when I did the analysis. Also the Urea was slightly high at 54 mg/dL. I believe that my triglycerides are elevated because my body is in ketogenic mode and uses them as its main source of energy and that is why they are elevated. The liver will be exporting VLDL and my adipocytes will be releasing triglycerides to the liver. I would like to know your opinion about this situation, but the truth is that my condition is better than ever. Thank you very much

Thank you for the great info! I have been watching a lot of your videos as I have been suggested to start a statin at age 33 because of LDL-C at 180-190 for two years (I’ve been LCHF for two years also). However HDL and TG have been optimal. I have not and do not plan to start a statin. I’m not sure if you have heard of “The Clot Thickens” by Dr Malcom Kendrick but it’s a great read with great information on the subject of CVD.

Thank you Dr. Ali. I think you just described what my problem is and best way to fix it. I had fatty liver, pre-diabetes a1c 5.8 and high cholesterol(high tg, low hdl, high ldl). I started keto, lost 50 lbs and BMI at normal and tg is now 65, hdl 48, ldl 220. My ldl is still at where it was prior keto. Lp-IR score suggest I am still insulin resistant. Fasting Insulin at 9. I am still not fixed my insulin resistant, so maybe that’s why I can’t get my LDL down. I am now trying Mediterranean keto diet to see if replacing saturated fat with unsaturated fat would help. If that doesn’t help, then I should lower my fat overall and do more fasting. But I don’t want to lose any more weight. What should I do?

I was watching your another video about glucagon not being suppressed, Can you please answer , if liver is insulin resistant and glucagon is present,why fatty acid cannot be oxidized and turn into ketones? while glucagon is performing gluconeogenesis and liver is acting like there is no insulin present. Appreciate your input.
