An elected judge of Knox Superior Court died during an RNAV/GPS approach to runway 18 in Lawrenceville (KLWV), Illinois. It was the end of a 22-mile journey from Sullivan airport (KSIV) and the middle of the night. This video details how the accident happened and what others can learn about risk management from this tragedy.

It's so easy to say "I would have done that differently..." but you never know if you would have made the go / no go decision the same as Judge Ryan. Condolences to his family and friends. Thank you for the detailed story telling. The recent Mooney crash was another low IFR approach dipping below the assigned altitudes for the approach. Those charts were written in the blood of many aviators that did not make it home. If I ever find myself filling up at 1:00 AM and the fatigue has set in, hopefully I'll remember to hop in the back seat and sleep until it's clear skies ahead.

Hence the need for stringent personal minimums and the spine to never bend them. I use a matrix that starts with “hours awake at landing”- over 12 single pilot and I stay on the ground. Prob 50 times in 14 years of IFR I’ve locked up the plane and grabbed a couch at the FBO. Never regretted any of them.

I'm a DPE. Once you pass your instrument checkride, you have the lowest possible qualifications to conduct instrument flight in IMC. PLEASE set high minimums and very gradually lower them; night IFR makes the whole thing much more difficult because you can't see inside -- or outside very well if at all. In particular, the transition to VMC when you have the airport/runway in sight sounds easy, but switching from instruments to unfamiliar outside references such as airport lighting in low vis can be very challenging.

Single-pilot, low IFR at night is as hard as it gets. This is no time to be even slightly unfamiliar or unpracticed with your avionics and automation. Only fly low IFR at night on a trip if you have recently practiced approaches under identical conditions at one or more familiar airports.

As a retired military pilot who also has thousands of hours in piston singles and twins flying privately, I would really avoid it. Throw in an emergency and you might have played your last hand. FYI, the FAA says piston engines fail about every three thousand hours.

@@FamilyManMoving Your number of engines can cover only one danger each. Over water, is a danger. Night, is a danger, IMC is a danger, Mountains, is a danger… Ice, is a danger… Never take on more danger than you have engines to cover the danger.

@@thatguy7085 Good points. I'm of the opinion that a single-engine piston is good for daytime flight, or clear nights in familiar territory. I might someday fly a twin, but until I had enough time in it I'd probably treat it the same. Engines won't fill in missing experience, after all. Best to know our limits. Thanks.

Not only beyond FSD on the CDI but more than 300' below glidepath and already BELOW MINIMUMS at point of impact. To have continued such a botched approach demonstrates a tragic and ironic absence of the most elementary standard of good judgement.

New alienautics, LATE night flight in IMC after a long day???...good judgment is essential for pilots...a beautiful morning flight the next day & home by lunch would be a better choice

Some lessons learned here: know your gear inside and out; get up-to-date weather on approach; don't fly night and IFR in a singe-engine piston at the same time. It's not fair to say, "shoulda", but as a student pilot I ask what I will do in that situation? 22 miles? I'd stay at the last FBO and sleep in the plane until morning. I've pulled over in cars during long drives and done the same when I or the road were in rough shape. I hope I won''t treat flying any different.

Sadly, it’s an all too predictable chain of poor decisions that leads to a fatality. Making 700 mile flight late in the evening when he was most likely tired, failing to recheck the weather enroute, especially in winder and darkness, third unfamiliarity with a new nav system, and fourth, failing to break off the failed approach and re capture the IAF on a new approach. Had he done any one of those things differently he would be alive today.

Unfortunately many don’t realize that ,hence why I fly vfr. If flying ceases to be fun I’m done. There is nowhere I need to be that badly . Great comment btw

Blow a cylinder or outright lose the engine and you're in real trouble...I'm not sure these guys are aware that the piston engine in light planes is mostly 1950s tech. They haven't evolved like car engines because so few planes are produced and sold.

More labor intensive..??..........Try a BE18 from 1960 with no autopilot/single pilot...........That's Me.......Takes lots of practice...Daily........Wide awake, alert, well rested and paying attention.........I've done at least 2 of these types/approaches with failed right engine feathered.........No go around likely......Serious business...........Paul

I learned that when getting there overrides safety nothing good happens, I got away with it with some shaky knees and a big kiss of some wet tarmac. I'll never again let my schedule risk my life. When in doubt, abort for another time.

Not ifr rated and haven't flown in 5 yrs but this is well done. Just got a pc w msfs 2022 & trying to fly an ils app has been difficult compared to earlier versions.

Single pilot night IFR is a serious handful….especially if the pilot is not intimately familiar - and proficient - with the avionics. Always one button push away from potential disaster.

Can someone help me understand this? When loading and activating Vectors To Final, at least a Garmin unit will intercept the final approach course outside the FAF. Am I misunderstanding something? Did this pilot load but not activate? This would have prevented descending down the (virtual) glide slope. I don't know if the Garmin sees a discontinuity or if it just turns onto the final approach course. The classic advice to always load an approach from a fix on the Garmin no longer holds. VTF will make the fixes available while allowing to intercept the final.

I am curious what gave the pilot the confidence for a decent when the aircraft would have clearly indicated it was off the centerline. I have personally made plenty of mistakes myself where I misinterpreted some information. What was the most likely source of confusion here?

I’d guess there was a navigational issue combined with fatigue combined with the expectation that he’d break out a thousand feet above the ground. Expectations are occasionally dangerous things…

A good example of why only professional pilots should be flying IMC at night. As systems become more complex, more time is required to learn them, and mist GA pilots don't have the time.

Sullivan was my home town, he would have better off to have waited till sun up. It's in country suround by corn fields he could have had a peaceful sleep and left in day light.

I installed a flightstream 210 in my plane. Made it simple to import my flight plan from ForeFlight. Would have likely kept him from the mistake.

This pilot must have just not been watching his approach on the instruments, Even when selecting the wrong type of approach one should have caught on right away, Pilots are trained to watch the instrument as approach changes come into play. I don't think the issue here was selecting the wrong approach it was not confirming each change in real-time, All pilots must keep an eye on (in no order) altitude, orientation, speed, and all approach changes all of these at all times, if you do this you will be okay. We must stay ahead of the plane, I hate to see good people die from simple mistakes, but in an aircraft, the smallest mistake is life or death, the good news is, follow some simple process and checklist you are golden.

I disagree I flew in the days of many buttons when 727 was new and every button/ switch had one job for the most part yes we had rotary switches but they had pointers they delineated what that function was. Today with equipment with two knobs it controls many different functions and unless you know the lay out or the architecture of the hierarchy of the switches you can make many makes! Just an old fart that knows new technology. 🤣🛫

​@@TheFalconJetDriver - If you read my post, I referred to GA aircraft. Not larger transport aircraft. This is significant as GA pilot don't have the extensive training ATPs do. They also don't usually have a co-pilot to help with the workload. I have had conversation with GA pilots who flew aircraft with what I call "video game" displays such as Garmin and Avidyne (as opposed to the old "steam" gauges). One pilot mentioned that he didn't know what "half the stuff" was or what it did on his aircraft.

Please do an analysis of his iPad and four flight updates. Was the iPad security breached? And was that part of the accident? With enough meddling into software, you could give pilots false data and create a very dangerous situation by taking attention away from the Garmin and towards the iPad. As we all know, the iPad has that ability to suck attention with its frequency, lights and buzzers... I'm 💯 against any blue tooth and cell phone technology in the cockpit...
