Recently I discovered my radiator cap was not working properly and letting the cooling system over-pressurize. This meant that coolant while under driving load was being dumped out of the over pressure hose and also leaking under pressure passed the radiator hose clamps.
When the engine cooled down, it then recovered what it had lost from the reservoir and thus, over the span of a month I was loosing about 250ml of coolant.
Simple diagnostics meant it was replaced with a new OEM part which stopped the issue but left unchecked, it could cause catastrophic damage and collateral damage to other systems including the engine.
