Louisiana Is Getting an Unlimited Supply of a $24K Hep C Cure (HBO)

Louisiana has struck a deal to improve access to a Hepatitis C cure so expensive that some people only acquire the drug once they already have liver damage.
Gilead Sciences, the owner of Hepatitis C cure Epclusa, will now provide an unlimited supply of the generic version of the drug to people covered by Medicaid and in Louisiana’s state prisons. Asegua, a subsidiary of Gilead making the generic, will supply the drugs.
The list price of Epclusa is $74,760; the generic is priced at $24,000. Medicaid receives a discount price, but in many states, including Louisiana, Medicaid only approves the treatment after Hepatitis C causes severe liver damage because of its astronomical cost.
Under the deal, dubbed the “Netflix model,” Louisiana will pay for the Hep C cure up to a negotiated spending cap in exchange for an unlimited supply of the drug over five years. The state will then receive a rebate from Gilead for all its expenses above that cap.
The Louisiana Department of Health first chose Gilead as a partner for the deal back in March, after three drug companies submitted proposals to the state. Since then, negotiations stalled, to the point that the agreement nearly fell through entirely. But this week, the deal closed, Louisiana Secretary of Health Rebekah Gee confirmed to VICE News.
Gee said she hoped to not spend more than $30 million - the cost of treating only 326 people last year, according to the state’s Department of Health. The pricing cap won’t be announced until next week, but Gee told VICE News that Gilead agreed to a higher amount than that. Louisiana will hold an official signing on the deal next week, according to Gee.
“We need to get our money’s worth,” Gee said. “Our goal is 10,000 [treated people] next year, but we’ll have to hit a lot less than that to make it work.”
Gee estimated that around 40,000 people in Louisiana suffer from Hepatitis C, a chronic liver disease spread through blood. It’s the most widespread infectious disease in the U.S. The CDC estimates 2.4 million people in the U.S. were living with Hep C in 2016, the latest year with data available.
Louisiana hopes to use the deal to attempt to eliminate the disease entirely.
As Hep C most commonly spreads through shared needles, the opioid epidemic has only made its prevalence worse. The CDC estimates that 41,200 people were infected in 2016.

Bless them and you sweetheart. I've been in the position also where I had a needed surgery that my insurance thought was "cosmetic". I was loosing blood to the point that I could have had heart problems, needed blood transfusions and many other horrible things but my insurance company thought this particular fix was not necessary. My doctor knew he wouldn't get paid, would have to have his medical instruments donated and would have to get special permission from a hospital that would allow him to perform this surgery on their insurance. All things that scared me half to death and I nearly backed out...The surgeon assured me that he could take care of me. 2 days after the surgery, I felt like a brand new person. Actually, I'm 45 but feel 18. I only had to pay $200 for my portion, surgeon incurred the rest. Still my doctor today😊

I'm from the UK and have just finished epclusa treatment that was free from the wonderful NHS, reading the comments from the folks in the US makes me appreciate how lucky I am and how much we have to fight the conservatives who are determined to privatise everything, God bless the NHS...

I went to pick up my new anti-depressant the other day and it cost me over $100 for a months supply. I HAVE insurance, but I'm still going to have to pay another bill every month to remain functional and not suicidal. It's new, so it's still under patent and there is no cheaper generic version. This is ridiculous and needs to stop. I'm lucky I'm in a position where this won't bankrupt me, most other people don't have that luxury. Come on washington; promote the general welfare of your people so that being poor isn't a death sentence.

The inventors of insulin gave it away because they just wanted to save people, but drug companies have been bankrupting people all over the country out of pure greed.

Government created the too policies that make them so high you know that right? This video is very vague about that. They have to go through clinical trials and failures to ensure government approval and that's partly why they cost so much because that testing and failing adds to the cost of developing the drug. While they definitely mark up the price beyond a reasonable price themselves, the generic price sometimes tells you how much they make it up by. 24k to 95k is substantial but even 24k is a lot of money being a generic, so the cure obviously cost a lot.

I was cured of Hep C by this exact drug, provided by a pilot program in Vancouver where I live. I had to wait a year until my liver started to show signs of damage, which was weird and scary, but Vancouver requires much less damage to qualify than Louisiana does, at least to my understanding. Total miracle drug. Little to no side effects. I was totally cured in just a few months. My health improved significantly. I'm so grateful to Canada's single payer public health care system. I would never have been able to afford to get my Hep C cured otherwise. For the all the problems with Canada's system, it clearly is superior to America's system. I would even be okay with totally privatized health care, no single payer, no public option, IF insurance companies were regulated in a way which encouraged competition, and IF pharmaceutical companies were regulated in ways that would prevent them from charging 91K$USD for a 12 week course of this Hep C drug. I am somewhat sympathetic to the fact that drugs take a lot of money and R&D time to create. Innovation could potentially be slowed if pharmaceutical companies were regulated and had price caps set on their products. But that argument is similar to the argument against rent control. In both cases of pharma and rent, I think that companies would still innovate new drugs and built new development even if their ability to charge exorbitant prices and make exorbitant profits were capped. Companies will still exist to service these markets if there is good profit to be made there - there is no logical argument for allowing gross, insane profit margins at the expense of society when those companies would provide the same services under conditions where gross exploitation was outlawed, but where a reasonable profit motive still existed.

Another crazy thing is that the office is right next door to a shell station that’s booming drugs in n out of it.. Actually that area is notorious for drugs, it’s sad.. But to drug company's defense, if drug prices were never high before, Gilead would never have funding to research a cure for HCV. Cost of drug development is insanely high (almost 2 billion) cause of high standards and regulations. Could do away with marketing/advertising though

What I don't understand about the U.S is that, something bad has to happen to the person in order to stop it. You have to be in danger in order to get real help? There was this guy who was getting harrased but a unknown person who knew all of his information but the police said they couldn't do anything because nothing had happened to him. You have to be in the verge of dying in order to get a small percentage of help.

I had to take a 6 month treatment of epclusa my doctor was able to get a cupoun from the company and i only had to pay 5 dollars compared to 23,000$ completly cured at the moment but it was defenitly scary i beleive it all depends on the doctors because i had one doctor say its going to be very expensive about 100,000 dollar ordeal amd then found another and the docter laughed and was like youll be fine i did look into going to India which is definitly a better option for people with out insurance but defenitly check out multiple doctors cause seems like some doctors dont have the right contacts as others

I used to live in Colorado and I have a lot of medical issues and ended up with an extremely large hospital bill and was unable to pay it and I ended getting hauled off to jail for medical debt because I missed a court date. Unfucking believable

well getting a brand new drugs market ready will cost a company up to 1 Billion $ somehow theyll need that money back. Research is immensively expensive and quite risky since faillure is always possible. Naturally a company will try to make as much money as they can. The only solution I see is government founded Pharmaceutical research. Esp. for this kind of livesaving medicine

With tuition free education, doctors and medical professionals will not justify millionaire-maker salaries and fees but rather could be paid reasonable salaries and will have zero education debt, helping medicine be about helping people again, not about the money with kids growing up aspiring to become millionaires and so choose to become doctors… The medical insurance industry will be freed to do something productive for society, as just one more example of our productivity gains.

While I agree meds should be affordable, a lot of meds wouldn’t exist without the incentive to make loads of money. I’m sure some scientists would rather give it away for free (or a reasonable price), but drugs take almost a decade or more to make it to market. They have to recoup the money they put in to making the drug(s). Plus, once the patents expire, the generic will be affordable

Haven't been to a doctor in 20 years minus a few ER visits to get stitches from work related injuries. Healthcare system is a racket. I just try to take decent care of myself. bassisku it’s a little more complicated than that. I agree that heath care is one of the industries that would benefit the most from being nationalized or socialized. But at least here in America the prevailing ideology is that individual responsibility trumps group responsibility. Littering would be a good example you could apply this principle to. So if you actually try to get something done you have to observe the culture. One way they could possibly fix the pressure from shareholders and stakeholders to squeeze as much money out of people as possible is to register as a B Corp. This business classification lends protection against retaliation by shareholders if the decision produces a positive externality or benefit the public over monetary gain.

Here is a depressing pharmaceutical patent story you guys in the US like to forget: 1- In October 2001, when there was the anthrax scare in the US right after 9/11, the US government pressured German pharmaceutical company Bayer to renounce to their patent and allow any US pharmaceutical company to produce as many doses of their anti-anthrax treatment Cipro as they wanted because it was an "emergency". At that point in time 9 people had been exposed to anthrax in the US. 2- At the very same time 40 US pharmaceutical companies were suing the south African government into submission with the assistance of the US Department of State because South Africa was trying to import ARV treatments for Aids from asian countries without paying the US companies for the patents. More than 7 million people lived with Aids in South Africa, but apparently from a US perspective that was NOT an "emergency". US imperialism in a nutshell.

Capitalism did not compell them, it just kept their life saving medicine from those who can't afford it...

The drug should be offered through medicaid first to those who got Hep C through no fault of their own, like the guy at the beginning. Once all those people have been treated move on to the drug users that got it through sharing needles. They made a conscious choice to risk exposure to Hep C when they started using intravenous drugs, my sympathy for them is very little.

Also, would it not make sense to cure the "high risk" people first if your goal was to eliminate the disease? High risk being people most likely to spread it. You may not think THEY deserve it, but you have to look at it in a bigger picture.

It's time we let money and GDP go as pretty much our only quantifier of success. Whether that be individual, business, national or global. In Bhutan they have an interesting concept. I'm not sure how well it is implemented (250 bucks a day to get in there) but it is called GNH or Gross National Happiness. They try to measure quality of life beyond monetary value and use that as their core goal platform. Wouldn't that be nice? Meanwhile these companies are literally holding lives hostage for cold hard cash. Yikes
