I have an hypothesis on the bending a control rod based upon long experience of working around morons. There was probably just enough play around the control rod to get at least a tenth of a degree off of square. Then rather than correct the alignment before proceeding the individuals tried to unstick the control rod with either sledge hammers or chain falls or both. I have seen this happen many times with hydraulic cylinder shafts and with shafts moving through linear bearings.

I know quite a bit about ARS. When you can honestly say severe ARS makes ebola look humane there is a big freaking problem.
