Posterior Tibial Tendonitis - What is it and how do you treat it?

Posterior tibial tendon dysfunction or posterior tibial tendonitis may present as pain at the attachment area of the tendon on the medial (inner) aspect of the navicular bone. However, pain can also present itself at the rear, inner (posterior, medial) aspect of the ankle bone, where the tendon passes through the tarsal tunnel. The posterior tibialis muscle may also be sore and tender to the touch.
Treatment options:
- Custom foot orthotics or arch supports
- Motion control shoes
- Strengthening, stretching and manipulation of the posterior tibialis muscle

Can this happen in a young runner that just recently started running more, like upping their mileage per week? I’ve never had this problem before, but i have had flat feet for most of my life and this just happened a few weeks ago.

Yes it could. If the posterior tibialis muscle and tendon aren't strong enough to withstand the added tension they will get aggravated. Keep in mind that you put 3x more pressure on the foot during running, compared to walking. You may want to consider orthotics combined with a strengthening routine or stability (motion control) shoes.

I've watched a few videos on the symptoms of this, and all of them match what I feel except that I'm not able to flex at the ankle without pain. None of the videos I watched mentioned this. Is that another symptom of this Tendinitis? Or could it be something else? I had Achilles Tendinitis that stopped me from flexing my ankle that I thought never went away, but just this morning I came to the realization that I think it actually DID go away, but just turned into posterior tibial Tendinitis because I've noticed when I try to flex the ankle it doesn't actually hurt the Achilles tendon anymore, but just on the inside of my ankle

Is it a pain you feel when you load the tendon only, or do you notice an electric shock type discomfort?

@FootGeekzTV it ONLY hurts when I try to stretch it at all. It doesn't hurt during any kind of activity, but I have noticed that after heavy activities I'm able to stretch it even less than before. And no electric shock feeling

If your arches are collapsing and the tendons are stressed, the pain will be present on weight-bearing and will linger at rest if you've been particularly active that day. If you also notice a burning or sharp electric shock type of sensation in the ankle, the swelling in the posterior tibial tendons may cause the posterior tibial nerve to be pinched (tarsal tunnel syndrome). If you have very low arches in non-weight bearing consider motion control shoes. If you have an arch in non-weight bearing look for well structured arch supports (such as Superfeet, Vionic or Currex Edge Pro) to counteract the arch collapse. Hi doc been having pain on the medial side of the ankle for 5 months now I’m 18 years old professional soccer academy player I sprained my ankle 4 times over the years recently I did an mri and it shows I developed ankle tenosynovitis is this the same as posterior tibial tendinitis or is it a different type of injury I can perform heel raises I don’t know what are my options to permanently heal im worried that it’s a permanent chronic condition since I’ve been having pain for 5 months

Did they diagnose which tendon is inflamed? There are 3 tendons that run or attach along the medial side of your foot (anterior tibialis, flexor hallucis, digitorum longus and posterior tibialis). Have you had inversion or eversion sprains? Do you have flat feet (arch collapse) on weight-bearing? When do you experience pain? Is the pain constant, irritated by activity or more like a tingling, numbing or electric shock type feeling (that comes and goes)?

@@FootGeekzTV I do have flat feet since i was born but not extreme flat feet there’s little arch and no tests were ran except for an MRI that says mild fluid signal intensity around distal part of tibialis posterior tendon suggestive of tenosynovinitis everything else is reported normal the 4 sprains I had were inversive except for one eversive the pain is tingling that comes and goes with activity it’s not as bad as I’m not able to do heel raises the pain isn’t killing level but it’s there can even be felt when I squeeze there Went to a doctor that told me I should just wear brace and wait for it to heal but it’s never fully healing

Based on your comments it appears that you may have some arch collapse on weight-bearing, which is most likely causing stress on the tendon, due to overstretching. I'd recommend using an arch support or ankle brace for your soccer activities to prevent the condition from getting aggravated, otherwise it won't have a chance to heal. If your arches are quite low, off the shelf arch supports may not work well and you may want to consider custom made supports. As for ankle bracing, there are select braces specific for posterior tibial tendon dysfunction, such as this: www.infracare.ca/shop/boa-afo-for-pttd/. Continue to work on strengthening the posterior tibialis, but be sure not to aggravate it. You may also want to perform more foot strengthening activities to build up arch strength.

does this condition limit my ankle range of motion? causing me to not be able to squat?

It's not likely to limit your ankle range of motion. Squatting may cause you pain in the inner ankle area due to a likely overpronated foot.

@@FootGeekzTV yeah thats what i have. but its limiting my mobility in my foot. i was put in a cast after a ankle fracture 3 months ago and i cannot progress my ankle range of motion because of pain i feel at the medial joint.

@@jeffreytheragene1763 If your foot has been in a cast for some time you will have to start with some mobility exercises to increase range of motion, as an immobile ankle will cause muscles to shorten and fascia to tighten. You may also have to get some fascia release to improve mobility.

@@FootGeekzTV yeah. but the problem is, my posterior tendonitis is the issue and causing me not to be able to improve my range of motion since when i try to extend of my range of motion, i begin to feel chronic pain behind the medial malleolus. so unfortunately until my tendonitis is gone, thats when i can get my full range of motion back.
