Reverse T3 Treatment Guide + How to “Flush” it out of Your Body

High reverse T3 levels result in a slow metabolism, weight gain, low body temperature and symptoms of hypothyroidism.
Reverse T3 is created from T4 in your body in the presence of inflammation, leptin resistance, insulin resistance and a history of chronic yo-yo dieting.
Taking T4 only medication can actually make high levels of reverse T3 even worse.
This guide discusses how patients get high levels of reverse T3, what causes reverse T3 to be elevated and how to flush high levels of reverse T3 out of your body. We will also discuss the normal ranges of reverse T3 in your body and how to evaluate them in the presence of low T3 levels.
There are ways to treat thyroid resistance but in some cases it requires the use of T3 containing medications.

It is good, but yes like to learn usual reverse T3 that is normal and would be expectedly. I take amour thyroid 90 mg. Units are important.

I’m confused because you say your rt3 levels were 24 but that seems really low. Idk maybe I’m wrong but I thought mine were low being 0.53

Let me add I have extreme fatigue, low metabolism, eat healthy won't lose weight, headaches, body aches ER blood tests showed low red blood cells, bigger than normal, low white blood cells, and low platelets but was not even told. I have researched myself. Also found out I have MTHFR GENE and two of the same c677T variants. She also never addressed this and I know it's a big deal. I feel im doing the work my doctors should be doing.

Do you take “methylated B12” & “methylated folate”? Copper/zinc & vitamin D levels checked? Good luck! About to see my third edocrinologist. Having hard time finding a doctor that cares. Hope you get better.

The problem I always run into regarding the doctors not knowing this stuff, including most naturopaths and homeopaths is not just that they don’t know but that they don’t want to know and are very resistant or dismissive to listening to a patient trying bring up any of this type of information. I have yet to find any doctor that has been willing to even give me the proper tests.

@@briks145 Same but I insisted then the lab didn't run them even after they told me I would have to pay and I said that's okay. The Dr had to ring them and say do the damn test - lucky they still had my samples.

Mine refused too, she said it wasn't necessary. After watching dr Childs, I asked her to check my cortisol and it was very high and I took dr childs supplements and it went down do fast. She never told me what to do about it. Or change diet so frustrating

Really solid video. We started looking into my wife’s health several years ago due to all the symptoms of hypo but normal TSH. Boy was that rabbit hole. This video encapsulates everything we learned in that time and I’m happy to report my wife was able to overcome high reverse T through NDT. This lead to increased energy which lead to working out more regularly and focusing on healthy clean eating. Mostly meat and fruit.

Thank you, Dr. Childs. I love that you talk about all this stuff that NOBODY is talking about. I saw 25 docs, took two rounds of Xifaxen and have been sick 3 years. Take a lot of thyroid medication, but I have RT3 Conversion issues. That part* Thank you for your ADVOCACY. Docs don't TALK about this stuff? I mean any of it. I didn't have thyroid medication for 18 months and was undermedicated for 27 months.

Thank you- hypo with flagged high IG-E Immunoglobulin Antibodies. I take three grain NDT, have used LDN 1.5 mg to come out of thyroid storm post black mold exposure, we lost almost everything. I had systems of organ failure beginning. 25 doctors did not know how to treat this- Southern California; THANK YOU FOR TALKING ABOUT THIS BECAUSE SO MANY PHYSICIANS REFUSE TO TALK ABOUT IT. I'M SO SICK OF being sick, fat, and walking infection control with Hashimotos.

Thank you very much for this video. I watched now a few of your, but this one is the most revealing one. I'm fighting with this issue since quite a while, got even told off from my GP when I was telling that I used google to find out about my symptoms where I came back with hypothyrodism and she replied that it was absolutely wrong as my TSH was normal. I'm quite worried on how less these doctors know these days. Actually from your list under "Are you pooling RT3" I do have all of the symptoms - especially the chronic pain is getting me as I have that in my hands and being a software engineer does not help there. Finally turning to a naturopath she told me to do an extensive thyroid test which revealed a ratio T3/RT3 of 0.0124 - slightly under 0.2 as you mentioned. Thanks to you and my naturopath, and a new GP who is willing to cooperate with my naturopath, there is a road to healthyness in this matter.

No to both. I did do a lifestyle change for 2 months, nothing progressed and still never lost the weight. I’m too selective with veggies so I could never go vegan. I do follow a lot of holistic thyroid doctors and some have said gluten free doesn’t always work either. I prob need to do a Dutch test honestly!

Amazing video. So very enlightening how it all works. I have experienced issues described. GP just raised Lethyroxine which I felt was overdosing(again). Decided to try help myself and reduced instead of increasing. Helped a lot, eyebrows back, not so jittery and sleeping better. But still not feeling good. Reverse T3 could be issue. I will follow all the advice outlined here that I can do without tests available, will monitor on how I feel and body reacting. This is very clear and I found on whole easy to understand! SO here goes! A big THANKYOU

THANK YOU so much for making such a detailed video about the issues surrounding rt3. I have been researching into this after experiencing unusual hypo symptoms despite not being diagnosed with a thyroid issue. I do have pcos and so always put them down to that. When I started experiencing unusual hairloss and eyebrows shedding I began to think it was a little deeper than just pcos. Can you see any abnormalities in the following thyroid bloods below MY THYROIDS RESULTS.: TSH: 1.8 miu/l (0.27 - 4.2) T4: 17.2 pmol/L (12 - 22) T3: 4.73 pol/L (3.1 - 6.8) RT3: 23 (10 - 24) FT3 : RT3: 13.39 ratio >15 Thyroglobulin Antibodies: 14.6ku/L

This was alot of great information! This will help me with my issues. I have normal T4 and T3, high rt3 and high cortisol. I have all the symptoms of hypothyroidism, crashing fatigue. I was put on levotyroxine and felt horrible. I Will ask my doctor abouth t3 treatment. And undress the underlying cause why I have high rt3 and where the problem is coming from. I wish more doctors where interesting in the root cause and diffrent treatmenst for diffrent patient. Not everybody have the same thyroid problem.

Low iron is huge in being able to raise t4 medication . If not optimal this can, when trying to raise T4, cause RT3 going up. Abnormal cortisol levels cause probs too. Thanks for other vital info as well

I'm going to a lab in my area that will do blood tests without a doctors referral. I am going to have a Reverse T3 blood test done this coming week. I do have a good doctor as far as she has got me feeling much better over the last two years. She has performed every test except for this one. I have low testosterone so I get pellets inserted for that, I'm on 180mg of NP thyroid medication, I had a total thyroidectomy 3 years ago due to undiagnosed Hashimoto's for 10 years. My biggest problem still is losing weight. I don't over eat, I eat organic, grassfed, etc and limit my food to a protein smoothie around lunch and then I eat dinner that consists of meat, vegetables and one carb like potatoes or rice but even that's not with every dinner, sometimes I will make multiple vegetables to avoid the starch. I've been working out again for 2 months straight with zero results. I'm hoping this test will reveal something because I try so hard and feel like I'm getting no where. I'm not giving up though.

Me too. There aren't many Endocrinologists in my area and the one I go to, who is supposed to be highly recommended is not very knowledgeable at all. I had to tell her about this Dr. and needing to try T 3 medication because my Levothyroxine wasn't working, after she tried to tell me there was nothing she could do for me and that I needed to go to my Primary Dr., Nutrionist and Psychiatrist. I was very upset with her, obviously. After I told her what this Dr. said, she sent me for Thyroid levels blood tests and after she read my results she immediately lowered my Levothyroxine to 75 Mcg n put me on Liothyronine 5 Mcg. So, thank you for helping me help my Endo Dr. help me!!!

A year ago I was diagnosed with thyroid cancer, about a 5cm mass was found on the right side of what used to be my thyroid. February of last year I had a partial thyroidectomy. When the results came back, they came back positive for Cancer (3rd Stage). In June I had the remaining thyroid removed and in October I had a small round of radiation. Since this whole nightmare started I have been on levothyroxine, started at 175mcg and now I am at 150mcg. It's been a roller coaster ride, feeling tired, moody x 3, depressed, you name it I have felt it. I have seen two endocrinologist one was very aggressive needed a second opinion. My current Endo refused to run the test that Dr. Childs has recommended to be done. I feel like I did when I got my first operation, so tired, moody, no motivation. My blood test results are not in range, they are all over the place. But again my doctor says to me, "No they are normal". This is not normal. This is not who I am and I want to feel normal. Don't know what to do anymore. I started the supplements that Dr. Childs recommends a week ago, I hope to see some kind of change. Take care everyone, be safe.

I hope you found another doctor. I wasted so much time with a doctor who simply wouldn't listen and only ran TSH. I'm doing better now that I found a doctor who runs all the tests I need. It took a while but I hope you find one too.

I also gone through the exact same thing ..it's been since 2012 and finally after many doctors , I found the one ..Please don't settle with one doctor or 2 .I felt horrible and I knew something was wrong ..Doctors would say the same thing..Your numbers are "normal "but I feel awful how is that possible,.had all those symptoms and more ,They didn't care to do further test so I ditch them and went with outside doctor and Yes ..He did a complete thyroid lab test and check my Rt3/ T3 and they were definitely off ..Kaiser won't do any other lab test other than the basic TSH. We need to check all the rest..I hope you find the doctor that will take the time to do the right test and you start feeling better once again ..

I am 16 months post total thyroidectomy today. I was placed on a secondary thyroid medication August 20th and everything has gone downhill rapidly from there. I have never gotten a full thyroid panel and requested it two days ago. Just looking at my symptoms I know that I have high reverse T3! It pisses me off that I have been a patient of this one particular endocrinologist's for 12 months, and he has never ordered a full thyroid panel until I demanded it. I had to be rushed by ambulance to the hospital two nights ago and my blood glucose level at that time was 124. That has never happened to me before and I have never been labeled as pre-diabetic, let alone a couple of steps away from having diabetes. Clearly that is showing that my body has excessive inflammation and so I am going to have my inflammation markers test redone because they were done right before I changed medications. Thank you for this video. I took copious notes and will look at them now to make sure that I understand everything you were trying to get across.

I am going on Cytomel because I am post ablation graves disease, and Levo has got my TSH in range but has never done anything else for me. T3 still low, Rt3 still high, and yes I have insisted on having these markers done because my doctor didnt understand why I would want them. Its nice to know taking T4 can increase my Rt3 altogether, so I am excited to try Cytomel. Thanks for the enlightening information. I am on a holistic path the past year and my life is in check basically other than figuring out this thyroid puzzle (Im 27 btw). Obviously I regret getting my thyroid ablated and if I hadnt I am sure I could of reversed my autoimmunity through lifestyle changes. I do intermittent fasting from any where of 16 to 20 hours a day (recently 20) and I basically eat at dinner for a few hours as I take my time. I have a huge appetite and have always been able to absolutely slam food. Back in the day I ate 6 meals a day because I was so into the bodybuilding bro science. Until I realized how inflammatory and stupid that was, I started intermittent fasting. Can intermittent fasting in general increase RT3? I NEVER intentionally calorie restrict because I basically have had a bottomless pit throughout my late teens into my 20's till now with graves. But I DO eat paleo and whole foods now (past year). I eat extremely nutrient dense foods, I will smash some potatoes or squash and fruits at dinner in addition to a leafy green and my choice of pasture raised meat for the day. I do not eat grains (no gluten at all), dairy or legumes. So I am potentially getting less calories than I used to, but way more nutrients....my question is basically how does your body respond to that? Because I used to eat a lot of bread, peanut butter, rice, cheese, etc etc. I have never counted calories because I have always been low body fat and never cared if I put on some weight, but I never could put on weight. I mean seriously, I could eat like a bulimic and stress the shit out of my digestive tract and never gain a pound. The only reason I do not eat completely to my hearts content, like lets say eat over a pound of meat at dinner is because I do or did have an atrophied kidney as of 8 months ago, and probably for a while before that. But I have done two 3 day dry fasts and dumped a ton of sediment for about a week after the first one. So I assumed my kidneys healed up a bit. But I stop eating meat when I am at about .8 grams of protein per my body weight in pounds. I am 5 foot 11 and 180 if that makes any difference. About 7 percent body fat. Hell I had a TSH of 66 this summer from refusing to take LEVO for 3 months (I had a theory I was still hyper becuase I couldnt gain weight still and felt the same as before ablation, YES I realize this was stupid of me). But I was still lifting weights and active, just felt horribly run down all the time. IF anyone can offer advice I would be eternally grateful.

I take T3 compounded from my pharmacy 1/4 the price! you have to get your dr. to ok that type of t3 though and mine did. It actually works better for me than Cytomel cuz it doesn't have added fillers, so maybe I was reacting to fillers...anyway...need more tests now

Hi Dr Childs, Thanks for your video and very useful. I will definitely get RT3 tested as my TSH/FT4 is normal but FT3 is low and I have Hashimoto's. You're right, GP is only interested in TSH and doesn't understand how to interpret RT3 even if they requested it. Assuming you lost all the weight you wanted to because you treated hormones, gut issues, inflammation etc - then your Leptin levels should come down as Leptin is released by fat cells and you will have less fat cells, so why would you likely gain the weight back again? The high Leptin levels were because one was chronically overweight/obese, so are you predisposed to gain the weight back again because of what your Leptin levels once were?

Thank you, Dr. Childs! It baffles me that conventional MD's refuse to run rT3 or even consider possible hormone resistance. You mention hormone imbalance (leptin and insulin) as playing a role in high rT3, but do other hormones (specifically sex hormones: estrogen, progesterone, testosterone but even vitamin D as a prohormone) also come into play? If all of these are low (in my case due to pharmaceutical injury), would that likely create hidden inflammation which then leads to hormone resistance? Anything to clean off all receptors? Appreciate your thoughts.

I did a reversed T3 test after being on NP thyroid with HRT and my hair got super thin, and my reversed t3 level was at 31.2! I had been off NPT for 5 months even on a small dose because it made me to jumpy. Now at least, I know where to go to try to correct this.

I love Dr child he probably has saved my life. But he is talking so fast. He honestly sounds like he is on speed. And I’m hypothyroid and it’s difficult to listen to him. My nephew was like that when my sister put him on Ridlen the poor kid was so skinny and hyper. And she was never violent but she actually slapped him across the face he was only 7! It has to do with how physicians are taught. What I am referring to here is simple physiology but it's the application of the physiology to clinical medicine where the communication breaks down. That's why I try to make connections between conditions doctors are aware of already (euthyroid sick syndrome) to make the connection easier to understand.

Thanks for the video. I think more information showing the transitioning to the dosage of T3 only would help a lot. How much do we titrate up the dosage and when ? Also, how is the dosage different if a person is taking T3 versus SRT3?

Hey there, huge fan for years. I was looking for the linked case studies. Pituitary tumor, hypothyroid, now finding out leptin resistant and of course high reverse 3, low t3. My endocrinologists have never ordered the necessary tests in the past. I am doing all the things I've always been a whole food eater, active, Intermittent fasting, sleep is well. Doctors also need to be testing for C-Reactive Proteins (CRP) & Cytokines like Interleukin-6 (IL-6), which will initiate Non-Thyroidal Illness Syndrome / FT3 Deficiency Syndrome / Eurthyroid Sickness Syndrome.

Keep in mind that when Deiodinase D3 is activated, firstly it is converting T4 to r-T3 but also converting T3 to T2. . Similarly Deiodinases D1 & D2 convert r-T3 to T2

What is the effect like for someone who has had gastric sleeve surgery? I have hashimoto's and had sleeve surgery close to a year ago. My weight loss compared to others is much less. Trying to get some of these under control so I can maximum the benefit of the surgery.

My RT3 is 34.5 and my free T3 is 2.1 so my ratio is really bad at .06 I only just learned of my thyroid issues and I am searching for answers as to what’s the best steps for me to take. Working with a functional medicine doctor as well will definitely follow up with her. Also my TPO Ab are elevated at 39. And ANA by IFA is positive so it seems like what I hav going on is autoimmune.

I'm in the UK and I did a thyroid + 12 blood test as I have hypothyroidism. Been mucked about by GP's and so called Endocrinologists for years as I never felt well on Levo. I was taking 200mcg's of levo when I did the test. I have posted some levels here for you to see and a comment would be appreciated. Total T4 = 93 ... nmol/L 59-154 TSH = *5.65* ... mIU/L 0.27-4.2 Free Thyroxine = *23.5* ... pmol/L 12.0-22.0 Free T3 = 4.2 ... pmol/L 3.1-6.8 Reverse T3 = 21 ... mg/dL 10-24 I also have high levels of Hasimoto's attributed anti bodies Thyroglobulin antibody = *189.0* ... UI/mL 0-115 Thyroid peroxidase antibodies = *172.7*... UI/mL 0-34 What it confusing me is the TSH AND T4 levels are high, my T3 is in low range and my reverse T3 is high range ... I have started to take T3 meds only, I had a blood test recently (Do not have the results values) before an orthopaedic op that showed mt FT3 to be high BUT my TSH had dropped by all accounts. I ran out of T3 for about 3-4 weeks and was taking T4 again and fell straight back to feeling rubbish!! I have since sourced some more T3 and started to take it again ... Could you possibly tell me whats going on here as the so called 'Endodrinologist' over here who says T3 meds are dangerous totally and utterly evaded me asking the question about T3 medication, T4-T3 conversion and RT3 and replied to me 5 months later with 'Some assays are different' and NOTHING else. Don't eat gluten and watch videos by Dr William Davis on RU-vid about why this is. Take Zinc and Selenium for hormone conversion. Take Magnesium, B12 and iodine as recommended by Dr Sircus too. he has videos on RU-vid. I too am fed up with GP's too, wish they would look at symptoms too not just blood tests and have some compassion.Have you done the candida spit test on RU-vid? it worked for me as i treated mine first before getting vitamin levels up first and removed parasites i had.

Dr Childs. I'm on compounded T3/T4. My FT4 level is 1.3 and FT3 is 3.8. My RT3 is 20. The ratio is 19. Should I be concerned and how can I fix this. Should I lower my T4 dose? My RT3 was under 15 in all my past labs.

My RT3 is 24.8, Free T3 is 2.5, and free T4 is 1.02 and I have multiple thyroid nodules and goiter. Any hope for me? Oh, and my TSH is 2.39. My Endo is wanting me to have surgery (total thyroidectomy) but I'd like to treat with medications to see if that works before I do anything drastic. Any thoughts Dr Childs?

i am hypothyroid. had a gallbladder attack. have lost 35 lbs in six months by cutting back on eating in an attempt to stave off any more attacks/surgery. i take almost all the supplements you mentioned. i'm feeling much better thyroid wise. when i go for next bloodwork will ask for the RT3 test, just curious now.

I have a lot going on and have learned a lot from you and like you said most doctors don't want to test full panel or even understand how to read it. My TSH and my T4 is low What does that mean? She didn't even address it. I truly feel it all started with prolonged stressed in my adrenal glands setting out high levels of cortisol and then jacking everything else. Lol I'm also making B12 but not converting it into active form. I'm getting 1,000mg but don't feel that's enough to replenish what do you think?

It could mean a lot of things. You'll want to look at free T3 and reverse T3 in addition to those lab tests to get a better idea!

I managed to get my doctor to run all tests. They’re all out of Dr. Child’s optimal range but within my doctor’s understanding of “normal” so he wouldn’t even speak to me about making a small change like going from levothyroxine to tirisint sol. He instead told me to find a specialist somewhere. I’m doing all the healthy living changes but am at a dead end about what to do next. There were some antibodies but I don’t know if that means I have Hashimotos so even figuring out what supplements to take is difficult. So much good information, but in my hands only, it’s rather useless. Any guidance would be very appreciated.

Do you happen to know of Dr.Madhu Budhraja in Little Rock, Arkansas I've been to him twice The first visit he put me on Armour 38/9 for Hashimoto's and a month later he switched me to synthroid 50 and liothyronine 5mcg BID but at this same visit he only ordered TSH on my blood test but I did request for the antibodies and RT3/FT3 to be tested I HAVE LIKED HIM UP UNTIL THIS POINT BUT NOW I'M A LITTLE CONCERNED BECAUSE HE ONLY WANTED TO LOOK AT THE TSH ON THE BLOOD WORK He has considered all of my requests up until this point: when I asked specifically for armor at my first appointment he agreed but it was causing filmy/cloudy vision and my joints were hurting so we switched at this last visit at my request..he suggested the synthroid/liothyronine.....however I don't know if I should stay with him if he only wanted to look at the TSH on my blood work ... What would you suggest? He is a very kind Dr and really takes time to listen to his patients and considers their concerns

Flushing out the r-T3 may be a bad thing. I restate that Deiodinases D1 & D2 convert r-T3 to T2. . T2 is produced by the Thyroid Gland in small quantities, and it appears that T2 acts in a not dissimilar way to T3, but it is the next deiodination to T1 which could be very important because T1 has functions that affect the brain.

Dr childs, im really good with my diet and strength training 3x per week. Dont feel so stressed as i did prior to NDT. but im battling to reverse IR which i think i have or atleast it feels like my body is locked into this condition. I have high RT3. Range is 0.09-0.35ng/ml. Mine is 0.27. T3 is 4.4 (2.4-6) has gone up while on NDT. Shall i ask my doctor for a small dose of T3 only with the NDT. Currently in 2.5 grains broken up into 3x per day.

😊I have been dealing with hypothyroidism, PCOS, and menopause. I go to my regular GP and she was treating me with levothyroxine. I talked to her about adding liothyronine and she agreed. I ran out of levothyroxine for about 2 weeks and only had my liothyronine to take. I noticed immediately that I felt better when not taking the levothyroxine. It was like a switch. It didn’t have me running marathons or anything, but I noticed that my whole body didn’t ache like before. I was able to do more things. I talked to her again and she’s treating me with liothyronine only. I was doing great until I got COVID, but afterwards I’ve not been the same. It’s been 5 months since I had it and I’ve had such joint pain that sometimes it was hard to move. I also have no stamina when I’m doing things. I wear out a lot faster. I even had vision trouble after COVID. I had inflammation on the macula of my eye and I had white blurry spots in my vision for 3 months afterward. I’m thinking I should find a endocrinologist. I love my GP and have been seeing her since 1992. I just think I might need someone that deals with the whole endocrine system. I’m tired of being tired.

My story exactly. I am going to an endo that understands little of this I think. Not helping me. My good doctor quit shortly after the end of 2022. I had covid twice at 75. I can't seem to stop inflammation, pain my eyes are awful. I have developed a brain tumor. I need some serious help and don't know where to turn.

Before I got covid I decided to lower my Levo on my own. I took half the dosage and felt better. After almost 2 months my body started to ache, joint pain, vision problems. So I started slowly increasing my Levo and just started feeling better and bam I got covid. It's been 4 months of long covid mostly in bed, gerd issues, etc. I'm thinking I unintentionally lowered my immune system by cutting my Levo in half. I also am post menopausal. So I'm not sure I did the right thing with the Levo. It sure felt better but by the 6th or 7th week...it backfired. Man I wish i could talk with you. Hope we both heal soon❤

T4 is mostly a reservoir for T3 conversion. So priority should be placed on T3.

Is it possible to have hypothyroidism AND high reverse T3? Can high reverse T3 cause hoarse and painful voice and even complete loss of voice? This happened to me as I took Levothyroxine for 4 months, as I got to the point of having completely lost my voice I stopped taking the medication on my own. I have my voice luckily back now again but of course I have many symptoms of hypothyrodism, but my doctor said that I'm allergic to Levothyroxine but he doesn't prescribe me the Liothyronine... I'm screwed :(
