There's a lot of choices that people don't think about that can be made PRIOR to being placed on hospice. I hope this video gives you a little insight into what I would do if I was being placed on Hospice.

I’m 56 and have terminal cancer in my lungs and liver. I had esophageal cancer that metastasized to my lungs and liver. I have made peace with the fact that I won’t be here for my kids weddings and children. My son is 32 and my daughter is 26. They both have good jobs and I’m leaving them with enough to be comfortable. I have put my hospice/end of life list together and have given it to my son. Thank you, Julie, for the good advice.

I swabbed my grandmother’s mouth with water on a regular basis, but I ALSO did it with wine! Even when she was unconscious! She totally responded non-verbally and I knew it made her happy. I talked to her and played her favorite operas. Did similar for my dad, but for him it was Waylon Jennings and James Taylor. I talked to him a lot! It was healing for both of us, I feel.

When my dad was dying from unrelated complications to the heart surgery he'd had, they had to keep him sedated 24/7. He was this way for five weeks until he died, and everyone thought he wasn't aware of anything. But several days after his surgery, I was by his bedside and noticed how very dry and cracked his mouth was, and I could tell he didn't get that way overnight. It was due to a lack of any oral care by the nurses. So I got a clean washcloth, wet it with cold water, and began wiping the white film from the inside of his mouth. I left the cloth wet enough to wet his mouth well, but not so wet that water could trickle down his throat and strangle him. That cool, wet water felt so good to my dad that he began a feeble effort to raise his head up, as if he was trying to get more water. And then tears began to run down from his closed eyes, but I don't know if it was because the water in his very dry mouth felt so good, or if he knew I was the one tending to him so lovingly, or if he knew he was dying. But I had a 'chat' with his doctor and the nurses in the CICU, and for the next four weeks until he died, they never neglected his oral care again. I have since made sure that my family knows if, God forbid, I am ever where I can't do or speak for myself they better make sure I get good (wet!) oral care!

I have end stage liver disease. They think it’s from medication I’ve taken through the years. It’s decompensated and I fell with the parameters of having 1 to 2 years to live. That was over a year ago. It’s getting worse and your videos have really been helpful as I make my final care plans. I want to die at home. I will be getting a line put in to remove ascites before I enter hospice like you recommended. I want to be as comfortable as possible. I’ve made a list of what I want and who I want with me. This video is so very helpful. I am going to die. Nothing can change that. I absolutely don’t want anything done to me that will prolong my death. I’m also adamant about not allowing anyone to take pictures of my dying process. I don’t have close family, but I do have a wonderful family of friends. My best friend from college and my best friend who lives on a farm a mile away from mine. She’s also a nurse. I’m confident they will carry out my wishes. Please continue to share your expertise on these videos. You are truly an Angel and I’m so very thankful I found you on RU-vid. ❤

I,too,battled end stage liver disease.Survived 4 hepatic comas and was given the same prognosis as you. I lived for 11 and a half years before my life was saved by my heroine,my organ donor. I just wanted to reach out to you to encourage you to consider transplant. Even if one transplant center denies you,you can always apply to another. Okay,that's all I wanted to say except I have prayed for you. As somebody who understands living with esld,I send you love and I wish you peace.

I have worked with HOSPICE. 3 times. Fantastic opportunity, but very sad. My first patient had cancer, he died in the night, he was cold. The overnight lady left him for me. I had to sit outside and wait. The lady that came from HOSPICE, sang a beautiful Irish ballad! After I thanked her for him. She said, oh,honey, it was for you...❤❤crying again now.....thank you again.....Sonoma California HOSPICE is the BEST!

As a 40 year ICU/Trauma/Life Flight specialty certified nurse, I will ask the hospice nurse what their recommendations are for the care of my family or myself. Nurse Julie is a subject matter expert of what she's talking about. The hospice nurse gave recommendations when my mom died and it was so beautiful. Nursing has fought for quality end of life since before Dr. Elisabeth Kübler-Ross wrote On Death and Dying. A great book by the way.

You are SO funny! Diet Coke from McD’s 🤣🤣 I now have great respect for how my Mom was treated at her end-of-life final days. I was privileged to be present with her and soothed her with that sponge when her mouth and lips appeared dry. I was also able to request oral liquid morphine when she became agitated or distressed. As for me, after facing a potential stage 4 cancer diagnosis (negative…whew….reprieve and reset), I’ve decided I want hospice that offers a menu of pain killers. I will die alone, so I wanna be tripping as I go! Thank you for this service you offer to all of us! ❤

Hello from Sonoma County, California. I am a retired RN w/ 40 years, 24 years in dialysis. I gave Hospice care to my mother in law July 2020. She was 99 years old and contracted COVID at the Alzheimer’s care home where she lived. We had her transferred to my sister in law’s home when she was put on Hospice care. "Speak to them like they can hear you..." This is important... But also remember this when someone is dealing with a terminal illness such as cancer, speak to them normally... My sister passed away from cancer, and when I was talking to her away from family, I asked what was one thing she needed right then. She told me to treat her as I normally would. She was tired of hearing how they were praying for her, and the she will get through it. She appreciated the praying, but no one treated her normally after she was getting to a certain point. I made sure that I talked to her normally and treated her just like any other day. I was there until the very end. I miss my sister. We had really good talks before she passed. Thank you Hospice Nurse Julie for all the great videos. The videos brought me closure to her death.

All good info. My 91 yo old mom just updated her DNR to have at my house and she completed her advanced directive. She wants no heroic measures taken. She’s lived and loved her life. The idea of someone banging on her chest for a prolonged period is not something she desires.

I watched two close family members pass away in hospice last year and shortly after, I discovered my own colon cancer, that had been presumed to be under control, had spread to my lungs and now I'm facing stage 4 metastatic cancer so issues of EOL care is very real for me now. I loved this video for its matter of fact approach blended with humor and touching personal perspectives. Thank you. Foregive me but I am choosing not to subscribe to the channel as I would prefer not to have more videos served up at this time but I'm so glad that they will be there when I finally choose to seek them out again. I remember when we took my mother to the hospital, the day before the passed, a doctor and another staff member came in and asked my mother what she needed or what she wanted the outcome of her hospital visit to be. My mother started talking about getting stronger to be able to walk normally, she wanted physical therapy. She was very hard of hearing, so she didn’t hear the rest of the conversation when they told me she would have to go into hospice on Monday (it was Friday). And I was thinking to myself, “Hospice! She’ll never agree to that!” She was blissfully unaware of her situation. The staff knew she couldn’t hear what they were saying, and made no effort to let her know she was at the end. I didn’t let her know either, because I knew how she felt about hospice. She thought of hospice as, “they are just waiting for these people to die!” To her, it was horrific. She passed away that night. I wasn’t there with her because I didn’t think she was near death, so I didn’t stay. Had I known, I certainly would have been by her side.

Love your list! I'm 71 and reasonably healthy, and I don't want CPR or Life Support, ever. I'm ready whenever the good Lord decides it's Time. I have a file with my favorite music. When my grandfather was entering hospice and my father was diagnosed with Parkinsons, my therapist recommended I get Five Wishes documents for them to help determine what they wanted at the end of their lives. I ended up buying booklets for my whole family to fill out, they're super in-depth about aspects of the dying process I never would have even considered on my own.

I'm tortured by not handling my sister's passing better (with the knowledge I've gained from your channel). Especially how our loved ones can still hear us even when they're unconscious. I wish I knew about your channel two years ago. Her condition turned bad very quickly. They gave her 3 weeks, but in 5 days she was gone. I thought there was more time! In hindsight, I was in total denial. My brain replays it all and tortures myself over things I could have done better. I am so grateful she knew how very much I loved her. I think a lot about seeing her again in Heaven. We will laugh and cry and dance and rejoice! She was my spunky little sis and I miss her SO much!! ❤❤

I went through something similar. The torture was so unbearable I went into therapy. My therapist said something incredibly healing... "You did the best you could with the knowledge you had at the time." I hope this helps you, too.

Please, don’t torture yourself. I lost my sister over a year ago. I’ve second guessed decisions too. She wasn’t on hospice yet. She was getting ready to transfer to hospice, but instead passed in the ICU. I had conflicting advice from the ICU doctor and the palliative care doctor. I took the ICU doctor’s advice. Both told me it could be hours or days. She lasted 3 days in ICU. I wish I didn’t watch this video.

My father was under the "care" of a hospice service. There were drugs, diapers, and a wonder lady to give bathes. We did the rest. There was no mention of the two hour pill option until after his death. My mom was very angry that her husband of 63 years had to linger and suffer for so long.

For me, ice cold Mtn Dew. 2 years on from CPR, heart attack, lived through it. Not easy, very hard recovery, still pain and limitations, including brain fog. Doc said he would have put serious money on me not leaving the hospital alive. I know what I would want at EOL, and will write it out and spread it around my family. I love that you take such comfort from The Office. My daughter is like that with Murder She Wrote. I’m a retired hospice nurse; I worked for my hospice for 25 years. My cat is dying - and much more slowly than my other cat did. He’s comfortable and happy and right now he’s pretty close. And yes, the human skills all transfer to animals. I went over my own advance directive in detail with my family a couple of years back. My granddaughter and one of my grandsons are RNs. They chimed in “Don’t worry Nama. We’ll pull the plug.” That is such a nurse to another nurse thing to say! We all got a good laugh out of it. I purposefully chose a geriatrician for my personal physician because geriatricians are less prone to over-treat. And of course we have gone over my end of life care wishes also. I’m super healthy right now, but I am 75 years old!

My grandma was in the ICU at the end and mostly unresponsive... except when they asked if she wanted to watch a certain Faux news channel. Then she got animated and protested 😒 Like, imagine thinking putting on the news is relaxing, regardless of your affiliation. Should be a policy to allow anything BUT that 🙄

Former hospice and palliative care nurse (12 years) here.. thank you for making these videos. It is important for the general public to be exposed to this information. I want to be at home, clean, comfortable and with access to a caring hospice nurse and a cna. My kids both know my wishes. Its in my trust documentation. Im also donating my body to the medical college and everything is set. I dont want them worrying about a thing. I witnessed too many families scramble around and suffer wondering what to do for their loved one.

I am 82 with cancer and have followed your advice and compose my wishes explicitly with a punch list as outlined by you. This is what I put towards the end I have been listening to Hospice Nurse Julie for sometime now and have learned a good deal about death and dying and I highly recommend her to those that have considered the dying process for themselves or their loved ones. Knowledge goes a long long way in this process and it has been an education in how little I knew when my wife died and very mentally uncomfortable in being ignorant of the process as many are. I also thanked the people in advance for there care.

Another thing is to make sure your loved ones tell you it’s okay to leave. Often someone dying needs to hear this. My dad passed away a month ago, I made sure our family did this and I believe it’s why he died so peacefully. He knew we’d be okay.

I have friends a lot younger that will follow what I want even if it doesn't fit plan. I followed what my 1st husban wanted not what plan. Same with my 2 partner. We all have different goals. My mother helped several of her friends who didn't have family left.so maybe you can count on friends if you leave clear directions

My personality is “Highway to Hell” by AC/DC and anything that I think is funny. I’d love it to be like theater and have the grim reaper making appearances. On top of that, no IV fluids and many of those things you say when it comes to medical. If I’m dying, I want to be me until I’m gone. My doc was putting in a hyper Al tube when dad was dying and and dnr. I had to rush into the hospital to stop her. She said it was her job to save him. But he did not want artificial feeding. After that, we put him in hospice. He lived 3 days.

Oh gosh! We forgot to play Duran Duran and Depeche Mode. We had Miranda and 80's movies and her British comedies. I sang to her and her husband and our friend talked to her and laughed. We took away her TPN and saline. We kept her safe and comfortable. Amy told me to not stop talking to and telling her about my pain. We talked and laughed and cried together.

Thank you for your list. Number 1 on my list is do not disrespect me by talking to me like I am a dog. I was so mad one day when I was in a nursing home. One of the girls came in trying to act cute and asked if I make yet. I lost it I told her to please not disrespect me and treat me like a dog. I am a grown adult and you will treat me the way you would want to be treated. I hardly ever complained about any of the girls taking care of me, but I did for that Yes. Number 1 on my list is, even if I am unconscious, you will treat me with respect and dignity.

I am so glad you stress the no IV fluids or feeding. My nephew and his wife think the doctors killed her Dad because he wasn't given food and water. I tried to explain how that is proper, but I doubt they believe me. At 74, I find your videos very informative and somewhat comforting. Most of us refuse to think of our lives ending as a natural and inevitable process. It is a reality we must all face at some point.

Share that info with your doctor and if you have a particular hospital you go to, make sure it goes with you there. It can avoid so many fights at the end of someone's life if we can pull out a paper that is signed by the patient expressing their own wishes.

Well said Nurse Julie! Currently going through this with my dear mother! (a 1952, RN, graduate). She’s had multiple TIA’s & strokes this last year. Only 93 years young! The “dementia’s from strokes are so different. She still remembers her family. Thank you! ❤🙏🏻

thank you so much for this Julie!!!!! I have been DNR for a few years now. I'm 73, in very good health (still mow my own yard and maintain my home). But yes, to everything you've said here...well, except for the Office. I'm not much of a sit-com person 🙂 But I do have spotify lists that will be playing when I'm leaving. We ALL need to thoughtful about our "exit strategy"...(a business theme, but appropriate for every human regarding their lives ending). Again, thank you!

The feeding tube is a trigger for me. My mom had issues with internal bleeding or having strokes words the end. The doctor after recommended hospice so we agreed to meet with a representative the following morning. We arrived at the hospital the following morning to find the doctor had ordered a feeding tube shortly after we left the previous night. To say I was upset would be an understatement.

@@user-fl3im1qy2o yes it was removed within the hour but she had been on it receiving nutrients all night. It prolonged the hospice stay to over a week which was pure agony on the rest of the family.

I agree with everything you said, except the office (yuck) and coke of any kind! Ideally I would want to be in hospice at home with my cats on the bed and my daughter Azure holding my hand. She knows that I want to be cremated and have my ashes comingled with my soul cat's ashes and then scattered. Thank you for this and all of your videos.

Ms. Julie, your depth of knowledge about your field is refreshing. I have not commented before, however, I do watch your videos often. I lost my spouse about three yrs. ago, which was the most heartbreaking experience I've ever had. Some time back, though, I watched your vid. about the patients' experience at the end of life and the many folks' perception of what the patient is feeling v. what they actually feel. It lifted my spirits immensely. The thought that my lady was likely in pain was so heartbreaking! But now, the thought that she was likely at peace w/ no discomfort makes me feel so much better. I loved her so much & still do. I also appreciate you for what you do and who you appear to be. Thank you for puttimg my heart at ease. God bless you kind woman!

Just want to remind you about the most most important decision we need to make before we die, and that is to receive Jesus as our Savior. I just wanted to let you know a little about what the bible says. We must be Born Again to see the kingdom of God. Everyone has sinned and fallen short of God's glorious standard, and all need to be made right with God by his grace, which is a free gift. They need to be made free from sin through Jesus Christ. Bible also says, its not by works should any one boast. So being a good person does not get you to heaven either. The only way to heaven is by repenting of your sins, and putting your faith in Jesus. Bible says, Whosoever shall call upon the name of the Lord shall be saved. So you see, there is a need for salvation to get to heaven, that is why Jesus died on the cross. He took all of our sins upon him, so that if we receive Him, we can have eternal life. We must be Born Again to enter the kingdom of God. You do not get there being a good person, or doing good deeds. Jesus also says, that I am the way, the Truth, and the Life, no one comes unto the father, except through me. So please make that decision, so that you can have peace and comfort when your time is up. Its the most important decision of your life. you can then have that eternal peace and assurance before you die. All you have to do is humble your heart and say with faith, I repent of my sins, and I turn to You. You said in Your word that if we confess with our mouth the Lord Jesus, and believe in our hearts that God raised Him from the dead, we shall be saved. I ask You Father to forgive me of all my sins, and ask You to fill me with the Holy Spirit. Its not about the words, its about your heart. The Lord will send His Holy Spirit to live on the inside of you. The Holy Spirit is Gods seal of salvation, and your evidence you have been Born Again. He will transform your heart and mind. You will become a new creature. All things will pass away, all things will become new. Its truly amazing! You will be filled with peace, love, joy, kindness, and patience. The Holy Spirit is the difference between religion and a relationship with Jesus. Its the most important decision of anyone's life. God's Word says that we are saved by grace through faith in Christ Jesus and not by our own efforts or works (Ephesians 2:8-9). Grace Alone. Jesus is the only way to heaven. Please believe me, and research for yourself. Once its over, there is no turning back. Jesus cares for you, and does not wish for you to be separated from him. So many people think that being a good person gets you there, and its simply not true, so please don't ignore. Today is the day of salvation. He loves you, and does wish for anyone to be apart from Him, but to have eternal life in heaven. Be blessed. I'm praying for you. Please ask Jesus to come into your life today, my friend, He truly loves you! Romans 10:9-10. "If thou shalt confess with thy mouth the Lord Jesus, and shalt believe in thine heart that God hath raised him from the dead, thou shalt be saved. Be blessed

I don't want to speculate. I'll find out for myself if there is an afterlife. What I would prefer is to just go to sleep forever. I would not want to go to a place worse than the one I am leaving. As far as the end of life, my father stopped eating in the nursing home for dementia, doctor asked me if I wanted him to put in a feeding tube. I was torn. But I remember what my father told me "I'm ready to go into the box." Which I think he meant coffin. So I said no to the feeding tube. He fell unconscious and died not long after that. I am thankful I didn't try to prolong the inevitable. I followed my dad's final wish. He had dementia, but had some awareness. And I didn't have to watch him lose all awareness and have a blank stare and mouth open like I've seen some residents have.

When I made out my will I included no IV fluids, for all the reasons you mentioned. My lawyer was very surprised and said I was his first client that had ever requested that. I explained why. Frequent oral care is critical for comfort. Do mine with Dr. Pepper.

No feeding tube, no intubation, I pray for the fastest death possible… my mom just died 3 weeks ago, she had a DNR and we talked many times about what she wanted, she didn’t want any feeding tube, still her hospice wanted us to do a feeding tube, we, her children said No, absolutely not. My mom didn’t want anything like that. My dad is dying right now and he also has strong DNR, I made sure she had a dry death, thank you so much, Julie, your videos helped me so much!!❤❤❤

I want exactly what you want. This was very informative. After seeing 4 men in our family die of cancer in 5 years, I learned from them. Two of them were in ICU for 6 weeks, recovered enough to have cancer treatment and went on to die later. One lived 9 years, the other 4 years. Both were on ventilators for weeks, and feeding tubes. They went through hell, and both said never again. When my dad died , I do remember he was on fluids for a short time, but as you said would happen at end of life, his lungs filled up and they had to aspirate him, which was hard to observe. Within a day they took away the fluids, and of course he died within a few days.

Julie, HOW FUNNY... I feel the exact same way about The Office!! And exactly the same episodes!! I am nearing my time, and I already have it written down for my family that I want any hospice people to watch my favorite episodes of NURSE JULIE!! Just the couple that address my particular disease, and, THIS ONE!! You nailed it... people might plan their funerals, obits, and what they want as far as a resting place, but no one really plans their actual death. I love you so much, and no, I am not afraid, thanks to you!! 💕🌷💕

My mother in law had Alzheimer’s, and then (out of nowhere) started having heart murmurs (?). The doctors started discussing Pacemakers. No. it made no sense when her mind was so disease.

Im so scared of losing those closest to me. My sister is 15 yrs older in her 70s, type 1 diabetes since age 8 and in stage 4 kidney disease. Not on dialysis yet, may choose not to do it. Starting to talk about her wishes. My bf is 75 and has copd and heart issues. Still doing ok, active but time tics on. My daughter has crihns disease since age 6, now 39. Hospitalizations every year. Had stomach cancer, caught early. Had hlh, made it thru. I fear her death as well. This to say your videos are helpguk to prepare me for reality. In a time when ill be so sad. My bf and i are writung up needs and wants re care atc. Your advice is beautiful.
