For real. I've seen videos of doctors complaining about residents complaining about this "because I went through it so it's not fair if they don't". 🤬 Which is a very sad mentality.

Forcing doctors to work while severely sleep-deprived can't be good for patient outcomes.

The laws requiring breaks for truck drivers should apply to doctors as well. If the government won’t allow a truck driver to drive a tractor/trailer rig without adequate rest, it shouldn’t allow doctors and nurses to care for patients without adequate rest either.

And he’s not even lying! I’m a scrub student and literally watched the chief resident fall asleep looking into the partnered microscope during neurosurgery (the actual doctor was performing the surgery and he was meant to watch). Anyways, the main part of the surgery is over and he’s stitching this person’s scalp back together when he mentions he got 3 hours of sleep after being at the hospital all night. Sometimes I think it shouldn’t even be legal for this to happen to them. Talk about the root cause of burnout!!

Its hard to strike as doctors, because you aren't just beholden to your employer like in most jobs, you are beholden to your patients. If you threaten a strike unless working conditions improve, and your employer doesnt cave, patients may die for lack of appropriate care. As much as you know that the real at-fault party here is your employer, you are watching people suffer in a way you could immediately fix. Even in public healthcare systems, striking for increased funding and provisions will usually put you in the positions where you know you are abandoning people. We need unions, and where we have them they are more effective than nothing, but the strategy and bargaining is HARD.

@@perplexedon9834 The flip side of that of course is that patients are already suffering and dying for lack of appropriate care because of the working conditions, so it’s “more obvious and severe but temporary” versus “less obvious and slower but forever”. At least for a strike there can be lots of advance notice and the opportunity to plan for minimizing patient impact. Nurses unionize and strike and have the exact same issue.

I get that sometimes there are medical emergencies that people can’t get breaks for, but the entire industry can’t just be exempt from labor laws. There’s gotta be some rule like “If you average over 60 hours per week over the past 2 months, you get quadruple overtime pay” or something, which still gives flexibility in case of emergency but a strong disincentive for administrators to schedule that.

except medical trainees salaries are limited by ACGME funding provided by Medicare and thus subject to congressional approval. There are regional variation but there isn't an infinite pool of money hospitals use to pay residents. Despite this residents make millions per year for their hospital in productivity and efficiency bonuses so the true cost is hard to calculate

I am a GenX doctor in the UK. Although I have some issues with GenZ doctors'conduct in the workplace, they have got this one right; they absolutely prioritise their personal well-being. EU legislation did help as well. It came into effect in 2009 and prohibits doctors from working long, unsafe hours (although the directive is on average as it acknowledges that some weeks doctors work more to complete their oncall shifts but they should work less when not oncall). Currently, in the UK, you can not do more than four 12 hour shifts in a row and you need at least 48h rest after each block.

Well in other countries the culture has shifted. It does lead to different problems since the number of doctors working at one time decreases. Combine that with a higher percentage of doctors wishing part time, on parent leave and you get a system where you have alot of doctors but not enough work gets done. This leads to long cues. I think the underlying problem is the type of work that we are forced to preform rather than the hours. Every country has its own problems, but I would summarise it as unnecessary administrative work designed by people who have never treated a patient (or facilitated doing so,like our amazing support staff). Yet somehow they have to power to tell us how to work, what to document etc. Great video though! Captured my experience as surgery resident in Germany quiet well.
